How does someone OD by SMOKING 2 hits of heroin?

I did try searching on this first and mostly I got alot of results about smoking...everything. My friend was telling me earlier about how he had to take his buddy to the ER a couple nights ago after he OD'd while smoking. He said that he had only taken 2 hits, (he determined this by the amount left on the foil and the 2 black marks on the bottom.) I think this sounds really strange. It's not a matter of low tolerance or a sensitivity that he wasn't aware of because he smokes H fairly often. I asked if he'd been drinking or taken anything else and as far as anyone knows the only other thing he'd done was smoke meth beforehand. I always thought that speed would somewhat defeat the purpose of the heroin rather than intensify it. I've only ever tried smoking heroin once or twice. It tasted awful and didn't do a thing for me, so I'll stick with the IV ROA. But does anyone have any ideas why or if that combination could cause an overdose? I want to know for my own benefit since I frequently combine the 2 drugs myself. The speed keeps me from sleeping all the time on heroin and the heroin keeps me from getting all spun out on the speed. (It also has the wonderful bonus of shrinking my pupils back to normal.) I may be dumb enough to have acquired this lovely habit, but I try to be careful and informed. (that's how I found bluelight in the first place.)
 
could be a list of factors, herion coulda been laced, he coulda had a low tolerance , the meth coulda been cut with somthing that reacts to herion , the herion coulda been really really good and was just to strong for him to handle.
