Opioids how do ER formulations actually work when you take them as normal

ok so i have 7x30mg teva morphine sulphate ER tablets. I do have quite a large tolleremnce because i been shooting about 2-3 bags a day (here in the uk a bag = 0.1g = 100mg. oh and its Aafghan no3 brown powder that you have to add an acid to it to make it dissolve. Anyway so i was wondering is how these tablets actually work when you take thme normal. Do they dissolve half of the tablet when i first take it like it would with a normal IR tablet but then stop after releasing the first half and then dissolve the second half say 5 hours later? or does it just dissolve REALLY slowly throughout the day?

I understand that thats too complicated so ill try and make it simpler

say i have a 30mg ER morphine sulphate sublets and i also have 2x15mg tablets of morphine. If i took the ER one and someone else took the IR one would the extended release one dissolve in 2 sections so therefore releasing 15mg morphine in the first hour or so and then 4 pr 5 hours or so later release thew rest of the morphine or does it release like 2mg at a time continuously so even if you took 60mg morph ER yuo are not going to feel the same effect as taking 2x15mg IR tablets untill a certauin time has passed and the rest ofn the tablet releases the other 390mg morphine.

Sorry i find it REAKLLY hard to describe stuff and need some help not wating my MST's because i ddrank like 600mg of orapmorph last night so im thinking i have a BIG tollerence.

Well it depends on the pill. Typically, they release about 1/3rd of the drug immediately, with the remaining 2/3rds being released over the course of several hours. But every pill has it's own system, but nearly all of them release only 1/3rd at first, not half. To elaborate on your example, 15mg morphine IR actually gives a HIGHER PEAK PLASMA than 30mg ER morphine, because 30mg ER morphine releases 10mg initially, and the rest very slowly. But if you are banging morphine and H, not to mention 600 fucking mg oramorph( That would have me feeling fucking amazing) well, I wouldn't bother with ER, I mean it would take an ungodly dose. I know for me, a 100mg ER tab was significantly weaker than 30mg IV morphine, but that was a long time ago...

And they don't allow prices man :( not to be an ass, just warning you!!!
 
Seriously though, check out pubmed, 15mg ir gives higher peak plasma than 30mg ER, though the AUC is Apperantly the same. If I recall, another study said 10mg IV gave higher peak than 60mg ER morphine...
 
what is this higher peak plasma level? i dont understand. can someone explain to me?
 
If you draw a graph of drug in your blood versus time, the IR will have a higher peak (more intense effects), the ER will last longer, but there's the same amount of drug released in total.
 
right ok, i hget you. so the teva brand 30mg ER morphine tablets are pretty useless if i cant defeat the ER system? They gel up when crushed n mixed with water coz i thought it was worth a go. Thge thing is is that gel even active at all because i could at least plug them that way.
