I feel your situation having been in the same boat railing heroin. I know how the green eyed monster will take everything from you.
First off anybody will tell you that your really gonna have to wanna quit when you quit otherwise relapse is a given, it might seem tough but if I was in your shoes I would have a last binge for the high so you can say goodbye and take away one of your doses a week until your down to 2 and then leave 2 weeks before going to 1 dose and then take it down 5 mg a week after that. Don't make your last binge your last night alive either keep the dose reasonable.
The reason I'm suggesting a long taper is because it doesn't seem you've been doing it long enough to have hit rock bottom and hate it passionately. So the best chance of success for you and your girlfriend will come from a strict regiment and
positive reinforcement from each other. Just to recap on the c0nzor taper plan:
Week 1- take away one 15mg dose, not morning evening or night though.
Week 2- take away another 15mg dose, again leave your breakfast, lunch and bedtime dose alone.
Week 3- take away the dinner time dose, leaving just your breakfast, lunch and bedtime dose.
Week 4- take away your lunch time dose, leaving just breakfast and bedtime.
Week 5- take this week to let your body adjust, start doing things you used to do before drugs, peruse old hobbies and try not to argue with your significant other as you'll both be high strung and trying to come to terms with life appearing to be dull in comparison to life being high.
Week 6- take away that breakfast dose, make yourself a good drug free breakfast and maybe get some vitamins to take to ease you mentally for the day having given that part of your brain the satisfaction of swallowing a pill.
Week 7- again just take this time to adjust to life around you and the new dynamics of your relationship.
Week 8- take away 5mg of your overall dose and continue steps above unless you feel at this point your ready to come off it.
Week 9- take away another 5mg and continue to reinforce a positive attitude to your opiate free lives.
Week 10- take the plunge, choose a day and make it your last day walking in footsteps 5mg deep and step on level ground.
I can't guarentee success but as long as your committed you'll be alright and your little family unit can begin again. If you need somebody to talk too about anything or questions to ask just pm me, let us all know about your progress, study shows those whole journal do a lot better in recovery. I won't say good luck as luck has nothing to do with it, so keep your chin up and your doses gradually lower and your headed in the right direction. Without having to go to an in-patient rehab...
