help please supposed to switch from oxcy to bupe suboxone in 40 hrs!

I get chronic migraines and I am taking about 40-50 mg of oxcy a day broken in pieces. I take it when I start to feel withdraw which is about every 5 hours. I don't think I can go a full 24hrs before starting the sub. I am doing it in the office wed thurs and fri and I am wondering how much the dr should start me out on and what to do tommorrow so I can make it to my appointment the next day. When do I have to take my last OC before the sub and what else can I take to help me while I am waiting? What will help on Tuesday can I take my morning dose and afternoon and still be good on Wednesday 9am to start the sub? I get so nauseous and major back pain extreme RLS sweating tremmors after about 12 hrs. I was addicted to everything for 13 years then sober 6 and now been addicted about 1.5 years. I feel so awful ready to graduate with my chemical dependecy counselin license and degree in mental health
I relapsed in school when we started delving into our pasts and years of trauma came back to the surface I just want to feel like my self again. Even in my sobrietyI never felt truly healthy. I want to stop wanting drugs to make things better. Please help.....
 
So would you say you are taking about 10 mg 4-5x a day? That is my understanding.

At a dose like that there is a few things to realize.

1. It might be easier to deal with the oxycodone w/d now than the buprenorphine w/d that you will eventually deal with, unless you remain on it for life. That dose of oxy is pretty small, the w/d should be tolerable.

2. Do not let the doctor put you on a ridiculously high dose of buprenorphine. Anything more than 2 mg/day will give you a worse opioid habit than the 40 mg of oxy you already take. And bupe w/d will be just about as painful and last over 2 weeks, compared to 4-5 days for oxy.

3. It is very unlikely you will get a precipitated withdrawal when switching to suboxone, the dose is not high enough to warrant much concern. If you begin to feel w/d within 5 hours, I would think 12 hours is plenty of time to wait. I would not make this claim for just anyone, but you sound like a person who will be fine at this point(seem to have fast metabolism, low dose). Also, even if you took all 50 mg at once, at the 12 hour mark you hit around 3 half lives and have around 12.5 mg in your system. That is under the agonist ceiling of bupe, so theoretically pwd would not occur in that situation.
 
Doctors will tell you to wait 24 hours because they don't want you to go into precipitated withdrawals. Are you taking oxycontin or instant release oxycodone? If you go into withdrawals after 12 hours like the way you described, I would take my last oxy dose tomorrow afternoon.
 
the longer you wait the better it'll work anyways but if i do it less than 24 hours after my last oxy dose i go into pwds
 
Ok so after 4mg I was still shaking like crazy sweating and freezing and really confused.... So another 4mg then I though ok I feel a little better! but I kept shaking still had a high COWS score so I topped out at 12mg I left feeling completely normal. I got home and it was like walking in my house for the first time in 6months like I could see more clearly I was motivated, no nausea, I hadn't felt so normal and great in as long as can remember. Now it is 6 hours later and I am sweating shaking ready to puke my heart is racing what the heck is going on?! Is my dose too high or too low I seriously feel the same as I did going in this morning! Btw I am enrolled in a one year program working through SIOP program starting group and individual at 5 days a week then 3 then 2 moving down to one after 4 months. I don't want to go in tomorrow and them take my dose lower and then it doesn't work or go higher if I am already too high ugggh this is so confusing! Thanks for helping I really just want my life back I miss spending time with my family :(

Oxycotin time release by the way and I wish I could have went Cold Turkey I tried it twice and my problem is not quitting I can't stay off I need this structured program to keep me inline for the time being.

I have been in your position myself. And I did what you are doing and got help. Congrats to you man! The reason that you are shaking and feel the way you do, is because it is like your first day of Suboxone use. The way it works, is that it fills the opiate receptors and makes your body think it has full agonist opiates in it. That first 12mg dose Im sure got you feeling much better from the sounds of things. I am prescribed 16mg a day. And they say that I should take it in the morning, but I don't take it all at once. I stare out taking 4mg in the morning, 4 at lunch 4mg after work, and 4 before bed. I did that for about 3 months then I realized I was wasting a lot of it. Now people will tell you that 16mg's a day is too much. And I agree with them.... it is too much FOR THE LONG RUN. But in the beginning when you are still physically addicted, and not past the withdrawl window, I would take the fully perscribed amount. Now I take a total of 4mgs a day. 2mg in the morning, and 2mgs after work. And I am FINE. I start my taper in 2 months. Because I have backed myself down to 4mgs, and my doctor does a 6-8 month taper I will be fine. I have actually stopped taking suboxone a few times through-out my treatment. I had to do it because I have low-T and they had to do it to find out if it had to do with the Suboxone. Coming off of 4mgs of Suboxone a day gave me an uncomfortable feeling for a few days, and within a few days I was good. I simply cut a small shaving off if I was feeling really bad, and it worked wonders. Then I would go 2 days without it.. then 3 days, and long and longer before soon enough I didn't need it, and I wasn't in withdrawl. Getting off Suboxone/Subutex is all about TAPER and you don't want to rush your taper at all. If you have to extend it, talk to your doctor. They are usually willing to work with you. Atleast mine was. But I can tell you this..... The suboxone SAVED MY LIFE, and I wasn't on a HUGE does of heroine at the time I was on a couple of dillaudid a day. But it was enough that I couldn't do it on my own, and just coming off of the small dose of dillaudid was much, much worse than tapering off the subutex. Obviously if you could just do it yourself and deal with the WD's you wouldn't have went to a doctor for help with the suboxone. I woudln't take the chance at messing up your schooling since you have come so far. Take the suboxone, and do it responsibly. You'll make it. TAPER, TAPER,TAPER!!!! If you ever need anything don't be afraid to let us know.

(While I wrote "SUBOXONE" through out my super long winded post... I myself switched from Suboxone to Subutex at the 6 month mark because the Naloxone was messing with my T levels)
