I have to go under general anaesthetic for a medical procedure and I’m not inclined to disclose my opiate use. How long should I leave between a dose and the procedure? it’s relatively minor not an operation or anything so I was thinking 12 hours between last dose and I should be fine?

If you're going under GA I'd be in inclined to be as honest as possible with your anaesthesiologist. If that's a problem then maybe you could say your friend gave you some morphine etc when you hurt your back and then they'll know you've had something on board.
Could be risky if you don't.

Regular user it’s an elective surgery perhaps I should give it a miss until I’m clean?

Or come up with convincing cover story.
If it was me I would 100% postpone until clean. Even then you'll have a degree of tolerance so I'd say something like you fucked your back up etc for a couple weeks and had to take quite a bit of dihydrocodeine while it settled. You know when they're doing the pre op chat with you.
