Opioids Grapefruit/Grapefruit Juice - How long before your opioid dose?

in my previous research I never got a clear answer to whether grapefruit solely inhibits breakdown and thus extends half life or if it inhibits conversion into metabolites.. or some form of both, leading to suggestions of questionable efficacy for many drugs (blocking codeine and tramadol, and apparently oxymorphone being a major contributor to oxycodones efficacy due to it converting some oxycodone into oxymorphone)

Alas, I’d say it’s a safe bet to allow time for grapefruit first 30 mins to a hour, and then dose to allow the grapefruit to do whatever it does to the liver enzymes,

If dosing both at once, I’m not sure if the grapefruits effects kick in time to fully allow it to potentiate.

Unless.. dosing grapefruit with or after allows in someway to delay inhibition in a way that makes sense, allowing the drug to kick in first and then let the grapefruit take affect to block further metabolism.

I have no idea, and I’ve just been lazy to not look it up but curious nonetheless
 
The method I’ve been using for years is taking grapefruit juice (I use Simply GF brand) directly with my dose. Then, I continue to take sips of the juice for 10-15 mins after the dose. I have found this has worked the best for potentiating the prescription. This is for IR though since 30 minutes is the time it takes to hit for me. For my OxyContin, I sip GF juice for the first hour (that’s when I start to feel its effects). I am on 20mg OxyContin 2x daily and 10mg Oxycodone 3x daily as needed for breakthrough. Hope this helps!!

Oh, yeah, so grapefruit/juice actually REDUCED the effects of pro-drug opioids (so codeine, tramadol, dihydrocodeine).
In my specific case, it's Morphine, which has been shown in studied to reach "significantly higher" blood plasma levels with grapefruit.

I've tried it today. So I drank about a pint of grapefruit just like 40 mins before my dose and then a second pint with my dose.

I am no chemist, I would ask Grok AI. I drink it plain. I love the taste though. Plus, you get 60% of your vitamin C from one cup! Win-win!

You know, I have malabsorption and have just been diagnosed as deficient in EVERY VITAMIN*. I have been drinking like 3 pints of grapefruit juice a day as well as like 3 actual grapefruits a day for 3 or 4 days now and I feel marginally less shit than usual. I wonder if the sheer volume of vitamin C in all of that means I have absorbed enough to make a difference :)
I drink the juice just plain as it is. I found it an acquired taste, but I do find myself starting to enjoy it, especially if ice cold.
Grapefruit itself - like eating just a grapefruit as is - is still too bitter for me so I add maybe a couple teaspoons worth of sugar to it.
I bought canned grapefruit for a change today; grapefruit wedges (in their natural juices, not syrup) and I REALLY enjoyed it. For some reason much less better even though there was no added sugar. I guess the juice soaking into it? IDK. But the can said it contained 4 servings and I ate the whole thing inc. the juice lol.

You will definitely notice the difference of the potentiating effects from “grapefruit cocktail” to “simply grapefruit”. Simply brand is 4.99 in most stores, or less. Worth the money, and helps keeps my pain away longer with the effects.

High dose vitamin C has always reliably made me feel miles better, I drink around 500ml of Orange Juice a day, or try get to about 250mg of Vitamin C.

Antioxidant properties and that may lead to further widespread utility across the body. Can’t remember if it’s been speculated to be any good for tolerance but seen things about helping withdrawals
 
What’s interesting is my tolerance has been the same since I started my chronic pain management therapy in 2020. In other words, I still get the same pain relief, and “high” with my dose now as I did on day one. I guess that’s why my PCP has never had a problem prescribing me my current regimen. Thankfully, I haven’t had to up my dose because of increase of tolerance.

I haven't had "grapefruit cocktail", only like whole, natural grapefruits, 100% pure grapefruit juice and a can that was literally just grapefruit wedges in their natural juice and nothing else.

Eating plenty of fruits and vegetables makes an INSANE difference. I was vegan for several months a few years ago (had to stop as unintentional weight loss triggered another anorexic episode) and during that time I was full of energy all of the time instead of being constantly exhausted, I was MUCH happier, I felt great and everybody was telling me how amazing my skin and hair looked.
It actually had nothing to do with not eating animal products but was because I HAD been eating unhealthily so when I went vegan I went from eating maybe 4 or 5 portions of fruit/veg per week to suddenly eating 7-13 portions every day. The boost I'm feeling from all this grapefruit has reminded me of that and I'm definitely gonna go back to eating like I did then...not the vegan part, just the large amounts of a variety of fruits and vegetables.
