Grafting a small Trichocereus top to a large root?

Some time ago I bought some very thick and hearty Trichocereus cuttings that had clearly been grown outdoors. I planted them indoors and they continued to grow but the circumference of the cacti decreased dramatically giving the cacti a 'bottlenose dolphin' type appearance i.e. a significant tapering in the new growth. I find this aesthetically unappealing and want to cut off the new growth and re-plant it so that I can grow a cactus of consistent width. Here is my question: should I simply cut off the new growth and let it callus before replanting or should I removed the middle of the cacti and graft the new growth onto the old root so that I can take full advantage of the accelerated growth the summer months afford? If you suggest grafting, how should I do this given the disparity in core size between the root and the 'bottlenose' portion?
