Goverment forms?? (opiates)

When prescribed subutex for detox - the Dr filled out this goverment form, which seemed to simply state that I was 'opiate dependant', and that was it. I asked what it was used for - but I don't remember getting a clear answer from her (mind you I was in my 4th day of cold turkey WD). I think she assured me it was highly confidential and only accessible to a particular goverment dept, no others and then she apologised, but she had to do it??

Now I'm wondering what exactly is the purpose of this form and who see's it?

Does anyone know anything?

It's a Schedule VIII substance in Australia, I assume she had to fill out a form for authorization to Rx it or something?
