Opioids From Methadone to Exalgo questions.

I had an accident that damaged alot of soft tissue/tendons in my foot. I was on methadone 40 mg for a couple years but in the last year has been Hell. It has not only affected my endocrine causing adrenal insuffiency but I just have been plain sick. I got down to 25 mg along with 32mg of hydromorphone. My Dr switched me to 12mg exalgo a newer extended relase. I'm now on day 6 and feel awful! I've been taking much more of my break through meds to get through the day/night. Has anyone did this switch or no much about it. Of course I will have to call my PM Dr. this monday to let them know what going on. Just wondering if anyone else has dealt with this. Or has been on methadone and suddenly had it turn on them.

I've never been in such a situation but have heard some anecdotal stuff about methadone turning on the rare, unlucky person or two... hopefully someone'll give ya something better in terms of a response though.

Chances are you are going through withdrawal due to your dose not being high enough. I believe exalgo is supposed to be either 12 or 24 hour time release, and 12mg doesn't seem high enough especially if you do the math.... 12mg/24 hours thats only 0.5mg hydromorphone per hour.

If you were taking 32mg hydromorphone WITH methadone, there is no way that 12mg hydromorphone on a 24 hour time release is going to be enough.
 
Thanks for the replys. I still have the 4 a day breakthroughs with the exalgo. It's still not cutting it. I know I have some withdrawl but really bothers me the most is that I started getting sick long before cutting back. Even bumping my dose didn't seem to help much. I'm assuming it's going to take some time to get the methadone out due to the long half lifes and of course rewiring of the brain.
As toothpaste said what I don't get is how methadone turned on me after a time so severly or what the heck is going on all I know it I'm coming up on a year of this hell and losing hope with it.
If anyone has has methadone turn on them I'd love to know more about it and if they were able to work through it.

Methadone does that, it's happened to me before. My pain management doctor rotates meds, so every so often I change usually in this order methadone --> morphine --> oxycodone.

Methadone is a great pain killer, however there are many drawbacks to it. How long it takes to kick in, the chronic dosing, what it does to your tolerance, it can create many problems.

You should speak to your doctor about opiate rotation so you can get some professional feedback on what you're going through and that way your doctor can titrate your dose to ease those symptoms.
