over the heart, clean the area well and shave to remove a thin layer of skin, then rub down with alcohol to remove dead skin/etc. wipe with a damp cloth and let air dry then place patch on. Dont do this if you have no tolerance or never used a patch before. If your new to opiates place on arm but be aware fent is very strong and suttle.
 
^no, it's best not to go shaving or removing any layers of skin as it will (in most cases) speed up efficacy of the transdermermal system - releasing the fent at a quicker rate. the OP hasn't said anything about abusing the system so let's not jump to conclusions.

the best place to apply the patches is the upper body region closer to the heart. biceps, shoulders and midsection of the back and chest are the best places. being closer to the heart and the upper body they have the best blood flow.
