Opioids Fentanyl Patch vs. Opana ER

First a little background. Currently on 25mcg fentanyl patch q48 w/o breakthrough meds. Have some concerns about the fent patch. Spoke with my PM doctor last month and he basically gave me some options.

In everyone's opinion, what would be the best combination to not only get good pain relief but also some euphoric effects when taking meds as directed.

Fentanyl patch q48 w/ Percocet
Fentanyl patch q48 w/ Roxicodone (no APAP)
Kadian w/ Percocet or Roxi
Opana ER w/ Perc or Roxi
Opana ER w/ Opana IR
Dildudid w/ Perc or Roxi

Other suggestions?

Id go with dilaudid with roxi. Opana ER turns me off now because of the new formulation. Doesn't dissolve well for many people and therefore they just shit it out and get no effects. Fent is good for pain....but bad for euphoria. Dilaudid is good for pain and euphoria. Screw APAP, total useless to have it in any medication. APAP is good for fever reduction...thats it.

If so dilaudid is useless unless you have no tolerance at all. Fentanyl is not euphoric and skyrockets tolerance, so that's also a no.

Percocet/roxicodone etc same thing. Just with Tylenol.

Opana is good, oxy is good, morphine/kadian is okay

Dilaudid would always be my choice, but I can't speak for what would be appropriate for you.

Don't worry about the formulation there is a way to get your oxymorphone.Just study your gum chemistry.The gum can be precipitated and then rest is the usual work-up.I take Opana ER myself.

I don't see how dilaudid with roxies for breakthrough makes as much sense because dilaudid is very short acting

But yea, like others said, you probably want to go with that combo for the good feeling effects

Otherwise probly OpanaER with either dilaudid or roxies for breakthrough

opana ER (as long as you can digest the stuff) should be good for constant and roxy for breakthrough. that's the route i would take. i would forget the other things.

ps - as far as getting the new opans in your system properly (taken orally), a good way to take it is by using some sharp scissors to cut the pill up into as many tiny fragments as you can, mix in lemon juice in a shot glass for about 4 hours, then drink the solution.

I take 60mg. of Opana ER at 8am, and again at 8pm. I have generic oxymorphone 10mg. up to 3 per day to take for breakthrough pain. This works pretty well for me!
