Ok so I happened upon some watson 50 mcg/h fentanyl patches. I know these things are really dangerous, but I have to ask the question anyways: given that I currently have a very low tolerance to opiates/opioids, would I be able to wear one of these patches without ODing?

Thanks in advance for your help guys.

You could wear it w/o an OD, yes.

BUT

you shouldn't. i take like 30-35mg oxy to nod proper, and although the novelty of a patch is nice, i would turn it down without batting an eye. Fent is not something people without tolerance should fuck with man.

I was in the same situation about 9 years ago. I found a bunch of 50mcg Duragesics and did some research on them and then just tossed one on. After about 3 hours I started to notice I had a massive headache, stomach was churning with acid reflux and I was very drowsy and delerious and nodding off. So in short it wasn't very pleasant.

If I were you I would cut the patch into 10 equal pieces if they're the plain non-gel patches, if they're the gel Duragesic's then divide the gel as best you can into about 10 equal parts, and then put them in your mouth and leave them inbetween your cheeks and gums. Leave them there for at least an hour and you'll start to feel the effects. Do it once at a time and wait at least an hour inbetween dosing again and you should be fine without ODing.

Remember the golden rule: You can always add more, but never take away.

Thanks man, I was thinking about using this buccal method, seems like a lot of folks do it with good results.

Given your experience with wearing one, I think I'll forget about that... for now anyways. I know that feeling-- not from fent, but from to much h... its horrible, absolutely horrible. I'd actually consider that an OD personally.

NOOOOOOO!!!! I wore a 50mcg/hr patch when I had the tolerance of an elephant. I was able to do near 100mg of opana. Wearing that patch was a no-go though, I had to take it off after about the 10th good pukefest. It took about 10 hours to get out of my near overdose because so much was in my skin. Run like hell man.

A 50mcg/h patch likely contains a few grams milligrams of fent. Leaving a tenth of one in your mouth for an hour is liable to deliver several hundred micrograms at once. Be extremely careful and know what you are taking. A sitter would be a very good idea.

I thought they only contained about 5mg of fent delivered gradually over a 72 hour period. Cutting it into 10 equal parts would leave one with .5mg per dose (of course it's not exact with patches but it's close). I was under the impression that .5mg buccal style wouldn't have the potential to overdose someone, but if you say that I could be wrong Amapola I believe you, just wasn't aware that that would be a dangerous dose. Maybe im just jaded by my tolerance.

^Yes you got it correct in that I meant milligrams not grams. Furthermore 500mcg of fent probably won't cause an overdose and the lollipops come in even higher doses. I was merely reiterating the danger and trying to give the OP an idea of the masses involved (and failing at it ;)). There is also the difficulty converting its strength in that the analgesic potency is often quoted at 100x that of morphine but recreational euphoria is lower and it delivers it's punch in less than a quarter of the time.

Perhaps I'm just too used to warning people off of fent. Cutting a patch into 10ths is definitely better than those who recommend halves. :) It just worries me that people with huge tolerances are prescribed and find relief with smaller patches than a 100:1 ratio would indicate.

when i used to pop just 3 lortabs and get sky high,, i once put on a fentanyl patch 50/mcg hour, i was in bed itching, throwing up, numb as hell literally, i had to throw it off....fentanyl = no tolerance =death, resp. depression, death ESP. if injected or sublingually

It is dose dependent. I have had IV midazolam+fentanyl during surgery and somehow managed to live. :)

Trade those bad boys for some viks or perc's. You'll have a more enjoyable / safer time.

The jelly Jansen patches were the best. Matrix absorption is bullshit and does very lil if you have no body fat

Considering this site is dedicated to harm reduction and the OP stated a very low opioid tolerance along with asking about the dangers of 50ucg/hr patches your post scares the hell out of me.

Where is the scientific data to prove your statement?

What if somebody out there with zero tolerance finds a matrix patch and takes your advice only to wind up dead?

I have next to "no body fat" and have been using matrix patches (European Janssen Durogesic DTrans brand) for years now..... starting out at 12.5ucg/hr and working my way up to current rate of 300 to 400 ucg/hr; and therefore feel I'm in a strong position to state that matrix patches most certainly ARE effective, strong as hell and have just as much capacity to seriously harm and kill people who use them in a foolhardy fashion.

Fentanyl patches KILL people. All the brands have the capacity to kill and most certainly there have been cases of folk dying from using 50ucg/hr patches when they have no tolerance or very little tolerance.... especially when they haven't been prescribed and the unlucky (/foolhardy) users have found/bought/been given them.

It may well be the case that for you personally the rates of absorption have felt higher/ actually been higher in the case of gel patches but this is a far cry away from you being in a position to confidently state that if you have little body fat and use a matrix patch then the absorption rate is going to be pretty small....

I just want everyone to be aware that the matrix patches most certainly are NOT to be messed around with as though they're nothing.

PLEASE be careful with ALL fentanyl patches, matrix or gel. I know it's been said before a million times but all the while people continue to overdose from fentanyl patches then it means that the message STILL can be reiterated again and again.

Thanks.

Ya I realized that too... I'm not planning to do nearly that much. The patch has 5mg, so if I cut up into 10 pieces, I'd be putting 500mcg of fent in my mouth, which would probably kill me. Am I right?

Maybe not, I dunno. They're still just chillin here, I obviously got lots more reading to do first.

Trust me if I thought it would kill you I wouldn't of given you that advice. What everyone is trying to tell you though is that not having much opiate experience and messing with fentanyl is a bad idea, however you're somewhat experienced with opiates correct? I mean you did say in one of your posts that you've used heroin before correct?

Do more research about them and if you still feel uncomfortable taking them then just get rid of em to someone or trade em for something else that you would feel more comfortable with. Fentanyl is very potent and should not be made light of, however I highly doubt that 500mcg of fentanyl will kill you.

Oh no I wasn't trying to knock you or anything. Ya I have much exp, have been using them for quite some time... I just don't have a habit anymore. And its always been h mostly, and not much else.

I was thinking that 500 mcg would be a fairly high dose though; I mean the patch when you wear it is supposed to release 50 mcg over the course of an hour, and that is supposedly excellent pain relief for an opiate tolerant person. So my thinking was that 500 mcg would be a monster size dose.

EDIT: I just read a reliable internet source saying that a good initial transmucosal dose for pain relief would be around 200mcg. I apparently failed to differentiate transdermal and transmucosal dosing.

So, just as Mora Fiend said, it is very doubtful that 500 mcg would kill me. He certainly knows what he is talking about, as I'm sure most of you are already aware.
