Opioids fentanyl mg for mg conversion to oxy

about to be getting some fentanyl in the form of 25 µg transdermal patches.... my usual dose of oxy/hydro is like 60 mg's, but i've taken up to 150 mg's and been fine... well more then just "fine", ya know, lol

i've tried to find out what the strength of these patches will be comparable to in terms of oxy/hydro but come up with no luck.... so after years of lurking here i once again need the ever important help of bluelighters knowledge so i don't end up OD'ing....

is my tolerance high enough to handle these patches without any problem? about how many mg's of oxy are they equal to?

Well there is a difference between oxy and hydro quite a bit of a difference actually so what's your tolerance like how much oxy would be your prefered dose to nod out?

it'll take like 120mgs of oxy to get me to nod out...
i actually find oxy vs hydro equal mg for mg, the high is just different. believe it or not i find hydro a "heavier" feeling whereas oxy will keep me energetic

[edit] I'm re-opening this to add a few more things, fentanyl patches are far more useful as a maintenance opiate than for actually chasing a rush, due to the very low theraputic index.

The reccomended method I have heard of for "acute" usage of patches is to cut the patch into a dozen or more strips and chew them slowly. Less is more.

If you are planning on wearing ONE of the 25 mcg patchs normally (as it is intended to use) and not tampering with it, my unprofessional opinion is that you should be fine. I am currently prescribed the 50 mcg fentanyl patches. Before I was on the patch I took between 50-60 mg of oxycodone per day. (My doctor thought I was taking a bit more than that so I was concerned just like you are, that my patch dosage was going to be too high.) The 50 mcg patch was pretty strong but it was certainly tolerable. It was nowhere near an overdose though. This is why I think you should be fine on a 25 mcg patch.

My unprofessional advice is to put the patch on early in the morning (first thing when you get up), as it can take 12 to 24 hours to take effect. The object is to feel the patch take effect while you are AWAKE. You should get to know the patch and how it effects you if you plan to sleep with it on.

DISCLAIMER: Any advice on here should be scrutinized with a grain of salt. Always use your own best judgement. In all cases when trying something new, you should have a babysitter that will watch you and call for emergency help if needed. The patch, as with all medication, can effect people differently.

Your doctor sure raised your tolerance big time by moving you over to the Fentanyl patch.
 
That's not neccassarily true. Fentanyl has never made my tolerance climb at a faster rate than any other opiate.


100mic Fentanyl IV = 10mg morphine IV
100mic Fentanyl Buccal = 5mg Morphine IV
5mg Morphine IV = 2.5mg oxycodone OR or ~2.5mg-3.0mg oxycodone Intranasal

Fentanyl can have a wicked odd cross tolerance, and I myself needed 50%-60% of the calculated dose. So, I would always make the 33% cross tolerance adjustment when calculating opiates. once you have dosed 3-4-5 times then you will know much you need.
 
Okay, so i know this has been asked a million times but i roughly snort 60 mg's of oxycodone a day and the last five times i dosed i snorted 90mg within a 15 minute period and i didnt throw up or anything i just felt nice. i'd say the perfect nod would be roughly 100-115mg for me before i started throwing up. i have been using for a year now on and off but for the last 3 months alot. i get minor withdrawals when i stop using, not unbearable, but they are unpleasant and annoying. i am about to acquire five 25mcg/hr patches tomorrow. i was wondering how much i should dose. im assuming one and to leave it on? my goal is for a recreational high lol there is nothing medical about this so i want to know what the good dosage to get me pleasantly very high is without OD'ing.
 
depends on what kind of patch you're getting, gel vs. matrix. Either way it's highly dangerous and there isn't going to really be a way to do it safely without risk of OD.
 
do you think putting on a patch tomorrow is too risky? im pretty sure since its just 25mcg/hr i would realize it felt too strong before any very very serious negative side effects happened in which case i would just take the patch off.
 
based on the chart kekio posted im sure i'll be okay with just one patch and from that point on i can feel out the effects.
 
Like I said, it depends what brand it is, and whether it's a polymatrix patch (Mylan, Malli, to name a few) or a gel patch (Sandoz)

If it's a 25mcg/hr matrix patch, like the mylans, I would take a pair of scissors and cut the patch in half, for 12.5mg/hr to test the waters.

It takes 13 hours after applying the patch to your skin to feel it, but it's strong its overwhelming. (the best way to take it, don't attempt to smoke, chew, or otherwise tamper with the patch in any way besides cutting it in half.

If it's a gel patch, with a reservoir containing a lot of fentanyl gel, do not tamper with it under any circumstances, as a lethal amount of fentanyl will be released.

Okay thanks alot TC i appreiate all the help im almost positid it is a polymatrix patch and will check tomorrow before altering it in anyway but i will cut it the way u said when i know for sure. I'll either post back here or write a trip report tomorrow to let you and everyone know how it went. Thanks again
 
I appreciate the chart sekio posted. When I first read this post, I automatically started to get up to go get my papers that come with my patches :)
~To TheLostBoys... I am currently on 100mcg. When I first started about a year ago, I was just getting to the point of percocet not working.
Fast forward to now, I get absolutely no pain relief ( Or anything else for that matter!) from doses any less than 90mg oxycodone immediate release.
This drug (for me) definitely put my tolerance through the roof.... after a while, of course. it did not happen overnight. I want to add that when I started on Fentanyl, it was 50mcg, and my doctor had to back it down because it was too much for me.
Things sure are different now!~
I guess my point is, to anyone out there reading this thread, considering trying Fentanyl-
Payy attention to the moderators and other blue lighters on the subject. I hear them telling people all the time how very strong it is. I wish I could go back to the days where I would get excited about a 10 mg hydrocodone or percocet :) It is a hard journey to get back to that place, I believe.
