They have to be officially Out or on IR for that position on your roster. Once they're upgraded to Questionable, you can't make any roster moves until you move them off the IR spot.

Yes, but they can't add free agents until they address those spots. Those players had O or IR designation and were upgraded to Questionable after being put on their roster's IR.
