Hi-Long time lurker-just signed up because I hadn't seen a thread yet on beating the time release well on this one. Since I'm new to posting, I apologize in advance to the mods if I do something wrong or post this in the wrong place. Anyway...
I'm a chronic pain patient (multiple illnesses, most of which I'm still trying to find an answer for but unfortunately Dr. House doesn't actually exist) but I am on a drug regimen thanks to a couple of complete flat discs in my lower back. Because I am young (23) it took a long time to find a doctor willing to even help me. This group is good though and currently they have me on Exalgo 12 mg and Dillies for breakthrough. I always run out of the dillies before the month is up though and while the Exalgo keeps the withdraw at bay, it doesn't do shit for my pain. Since hydromorphone barely does anything orally anyway, I usually economize my dillies by using em nasally. The exalgo though, even when I get em apart, the white part of the pill is too goopy and wet to be useful in snorting. Tonight though I thought I might have more luck plugging and it worked perfectly.
I put the pill in my mouth and hold it in my gums for a good long time, usually it takes at least twenty minutes to get the pill soft enough to bite it in half right where the white and gray discs fuse. Then I used a kitchen knife to peel off the plasticy coating from the white half and scrape all the white stuff off of it into a small container. I tried to powderize and break it up as much as possible, but again that is NOT easy. I used some sterile water (about 6 or 7 ml) from a needless syringe to cover the powder and mix it up as best as possible. Water was room temp. Most of it dissolved but there were still some chunks but the bits were small enough to still be pulled up into the syringe. I then plugged and lay on my side for about 20 minutes and it kicked in beautifully, just as if it had been regular dillies. I know some are uncomfortable with plugging (I certainly don't find it a fun experience hah) but if it works it works. I also know most people think using hydromorphone any other way than IV is a waste, but A. trained doctors have a helish time finding my veins, once when i was in the hospital it took them twelve tries to get an IV in and in the end they had to call an anesthesiologist to numb my arm locally so they could dig around in there. B. Injecting is a line I personally just won't cross.
