Diamorphine Tablets

from what i have read / heard it only comes in ampouls/vials which makes sense when you consider diacetylmorphine's very low oral bioavalibility

p.s for the addict program it only comes in vials of that i am sure , in the netherlands they even make special vials which have caffeine in them for smokers (very concentrated)
 
If oxymorphone (aka Opana ER and Opana IR) was available, I would HIGHLY recommend it as your next choice. The ER formulation lasts a TRUE 12 HOURS when taken orally, and even if you crush it and insufflate it, you are looking at 4-6 hours of pain relief and euphoria. The Opana IR tablets can easily be injected, and for most people, 10mg of oxymorphone in an injectable form would last 3-5 injections, each injection giving extreme euphoria and lasting 3-4 hours (a -morphone that has "legs"!). It is very fast-acting when taken orally as well, the effects being felt within 20 minutes of consumption. When insufflated, the effects come on over a 10-15 minute time-frame, and you can feel the euphoria building quickly. It is THE drug of choice for chronic pain (I am a CP patient as well, but in the U.S.), beating out hydromorphone, oxycodone, morphine, and even diacetylmorphine in both euphoria AND pain relief.

However, as it's not available in the U.K., at least as far as I know, the Diamorphine tablets would probably be a good choice as a breakthrough pain medication, as I assume they will "kick in" faster than a standard Morphine Sulfate IR tablet due to the dual acetyl groups. It's possible, though, that those groups are broken off via metabolization in the liver or by the acid in your stomach. If that's the case, then I don't see it having much, if any, advantage over morphine (unless you break them down and inject them). I would talk to the doctor about it, and see if you can get something like Hydromorphone-Extended-Release and then the Diamorphine tablets for breakthrough pain. I can see that being a very effective combination.

First I've heard of that. Not that I'm a smoker anyway but I could probably be persuaded under the right conditions.
 
i would think oral diacetlymorphine(heroin) would be pointless because after it has been in contact with your body fluids for alittle bit of time it would convert back to morphine, so it would really be the same as oral morphine.
 
Chimpo is correct that the advantage to diamorphine is negated with oral consumption. Diamorphine is designed for SC usage.

Suprisingly, there is also an immense amount of ignorance in the UK over its usage. Most hold that it is not sedating and only designed for analgesia which seems to me to be a boderline retarded attitude.

As for 6-MAM, which the OP also asked about, it is not produced for usage. It is difficult to make because morphine must be selectively acetylated which is very difficult to do. IF you use a very weak acetic acid and adjust the time you can make it, but as I said it is not a commercial product.

Huh, I didn't realize Diamorphine was actually prescribed for pain

Makes sense that it's not very common, like a last line choice, with that acetyl group science
 
Even if you could get it RX'd, you'd have a hell of a time finding a pharmacy to fill it. I'm not a fan of Morphine so i probably wouldn't enjoy it, however.. I'm sure it works great for pain which is my main concern so I'd try it if i could.

I wonder if the tablets can be IV'd safely? Then they would work really nice.
 
The only problem is they only come in 10 mg strength. Considering I'm on 25 mcg/h of fentanyl now, this works out as about 90 mg morphine daily, and I need to go up to the 50 mcg patch, I'd have to take a lot of tablets daily... It could help for breakthrough pian though.

I think its funny that it says "No other names are used for Diamorphine".. . . . ......what about Heroin?

He asked about 6-glucorinide-morphine, a metabolite of morphine (I know you know this rachamim but others don't ;)) and linked to an ADD thread talking about how they may be using it for analgesia. Apparently has less side-effects then straight morphine, but it may not be available for a while depending on what phase of drug trialling it's in currently.
