As is typical of articles in Journal of Analytical Toxicology, there’s much technical information concerning the measurement of drug levels here that will be of minimal interest to clinicians. However, the paper also contains 3 fascinating case histories describing fatality associated with methylone.

Methylone is a cathinone derivative often found in “bath salts” and other similar products. It potently inhibits norepinephrine re-uptake, and also promotes cellular release of dopamine and serotonin, as well as norepinephrine. Its effects are similar to those of methamphetamine.

  • Case 1: A 23-yearold man was found walking into traffic, banging on cars and shouting obscenities, as well as “yelling out rap song lyrics and speaking different languages.” In the emergency department he was combative and required 5 staff members to restrain him. He was hyperthermic (105.9oF) and hypotensive. Despite treatment that included cooling and fluids, he developed metabolic acidosis, rhabdomyolysis, acute renal failure, seizures, and DIC. After suffering multiple cardiac arrests, he died 24 hours after admission.
  • Case 2: A 19-year-old woman ingested a pill known as “molly” at a concert. After suffering 2 seizures, she was asystolic on arrival at hospital. An axillary temperature was measured at 103.9oF. She could not be resuscitated.
  • Case 3: A 23-yer-old man went to a club and ingested a substance believed to be LSD. After he began acting erratically, club security personnel tied him to a chair with plastic food wrap and placed him in the back of a van. He was found seizing in the van 3 or 4 hours later. On arrival at hospital, his core temperature was 107oF and his blood pH 7.04. He died 45 minutes after admission to the emergency department.

Features common to these cases included severe sympathomimetic signs and symptoms, including hyperthermia, seizure activity, and metabolic acidosis. Blood samples of all 3 patients were positive for methylone, but negative for mephedrone, methedrone, and MDPV. Although it is not clear from the paper, apparently no LSD or methamphetamine was detected. Extensive laboratory results are provided for Case 1 throughout his 24-hour hospital course.

Wow @ Case 3. This is why we need strict regulations on who is allowed to be a bouncer and what they are allowed to do. Fucking steroid munching gorillas.

Apologies for the derail. Great find nonetheless.

This reminds me of the time I did a little too much MXE at a show. The venue turned off the water fountains to force people to buy water. I ended up throwing up in a cup, completely in a cup, not at all on anyone else or on the floor. I was not causing problems at all, I wasn't belligerent or fighting or even saying anything to anyone really. Security forced me to leave without making sure I was okay or anything. They kicked me out and did nothing to make sure I wasn't going to die-not to say I would have but they didn't know that. Luckily, I attended with friends and they helped me out.

Basically, usually, security is really fucking shitty in my experiences.

Wow, great find, that is obscenely hyperthermic. Not too surprising, whaddya expect when you cross MDMA and a cathinone har har har. . .

Not to derail the thread topic, but security is a joke nine times out of time. At almost every party I go to in NYC, security confiscates found drugs, and pockets them, even going so far as to openly saying "Found some! Let's go push this". I shit you not.

yeah security is shady at every event/rave/club i have been to ime, other than some outdoor events where there actually wasn't any security at all basically, didn't even need security was barely any people and no one was out of control all weekend.

hell i actually have bought legitimate mdma tablets from security before, i knew him before hand but still its shady business on his part, i actually got good quality medium dosed tablets from him(im in the us of course).

the putting the guy in the van clear wrapped is fucked up that's a bit creepy imo. makes me wonder how many others were thrown in vans when they were to high.

They must've fair OD'd! Seems to me like these poor naive fucks don't get the info they need about drugs. With rc's & internet they can easily get hold of them withough needing to know 'the right people', people who would likely give them a few pointers regarding the dangers and responsibilities.
We grow up in this ever-protective society and people get more and more naive, and act like they don't have to worry and everything's taken care of for them, but real dangers will always lurk in life - more fool the sheep.

this shit is the truth. How easily we forget that we're just machines that can break down as soon as one little thing goes wrong. Break down beyond all hope of repair.

This is consistent with my personal experience with a high dose of Methylone which led to rhabdomyolysis. Sadly this condition is rarely diagnosed correctly. My urine sample was taken to late. My descriptions of my muscles becoming soft and the water I drank diffusing into the muscles around my stomach were't taken seriously. Could have died in the hospital.

Did they test any of these people for PMA/PMMA? I know a PMA/MDMA cocktail can be deadly: no reason to suppose a PMMA/bk-MDMA pill would be any less hazardous to your health.

As far as the idiots who thought restraining somebody with plastic wrap and leaving them unattended for hours was a good idea, they should be in prison for negligent homicide at the very least.

FoxyFox: how much methylone was involved in your overdose? I have never taken much over 350mg at one time or 500mg in a session: I find that for me higher doses just increase the peripheral stimulation without providing any more of the enjoyable empathogenic effects. But I have seen reports of people taking much larger quantities. (I've also seen a few accounts of people IVing methylone and claiming it provides a great rush: I could see where someone could get into a dopamine-induced lever-pulling frenzy and start banging every few minutes until they hit toxic levels).

I've always been under the impression that methylone was one of the more benevolent compounds to get lumped in with the "bath salts" hysteria: it's certainly far less more-ish (for me anyway) than mephedrone and far less likely to send you into week-long stimulant psychosis than MDPV and the other pyrovalerones.

This doesn't sound at all like methylone. I've used it a lot and it's always felt somewhat mild or gentle in comparison to - say - mdma. I say mdma because the first couple times I tried methylone, I was convinced it was moderate purity molly.

However, like everyone's heard a thousand times before, we're all different, we don't all react similarly to any given psychoactive drug and much of the time - when it comes to street drugs such as molly in capsules bought at a rave - we do not know how much we're ingesting. Sure, some peeps might be able to eyeball a dose but I take it most won't.

Unless the deceased each ingested massive doses, I cannot help but suspect that there is more to the story of each individual. I feel particularly bad for the 23 year old man who was tied to a chair with plastic food wrap and placed in the back of a van and left there to rot. An extremely unpleasant way to spend your last few hours for sure. Whoever did that to him should be behind bars.

These look to me like pretty serious overdoses on methylone; I don't think this is very typical for people who use it responsibly.

^ The high body temp was probably what caused e.g. organ failure, yes

Generally MDMA/methylone are not toxic on their own - it's the secondaty symptoms like overheating and electrolyte balances that do you in

Lets not forget that people can die from MDMA too... methylone seems to have much more peripheral side effects as well, as well as seeming weaker, leading people to dose more and more.

These deaths don't sound far fetched to me at all, especially since none of these people took the drug as Methylone, they were sold it as another substance.

Case three seems atrociouse.... There should have been many different ways staff could have restrained him other than a cruel mechanical strait such as a chair and plastic wrap....
