I've been taking Oxycodone daily for 4 years for chronic pain but in the past 8 months my dose has tripled and I found myself addicted. I was taking 65mg Oxycodone liquid and now I'm trying to stop but I started having withdrawals and I ran out of my Oxycodone precription within 3 days and I cant go back to my doctor for another week for another script.

But I have a few boxes of 12.5mcg fentanyl patches left over. I stcuk one of them on my skin for 2 days but it barely helped with the withdrawals and instead it just gave me a massive headache. Reading online I heard some people are cutting them up and chewing / sucking on them. I was wondering If i wanted to do this how many strips would I have to cut a 12.5mcg patch into for it to be equalant to 65mg Oxycodone?

Hi. I have cut up and chewed fentanyl patches that were 25-75mcg/hr. I got very, very sick when I chewed and swallowed a cut up 75mcg/hr patch. I don't know the equivalent doses but would suggest you cut strips and wait for some time before taking another one. Be careful with fentanyl. Good luck.

Thanks for your warning. I know it can be dangerous thats why I'm trying to determin a dose for the cut strips. I'm just going to chew one strip a day and cut down from there but first I have to work out how much to cut up. Should I cut a patch into quarters or even smaller 8 parts? I have no idea at the moment on dose , so I am hoping someone who has cut a 12.5 mcg patch would have a rough idea

If I remember correctly, a 12.5ug patch is total about 2mg. I'd say you wanna try no more than about 0.5mg chewed, so just cut it up into quarters. If that doesn't do it for you, go up to about 1mg (though give it some time to kick on of course, fent ODs tend to be worse than most other opis)

If you are concerned about an OD, get some Narcan (naloxone) and familiarize yourself/people around you with how and when to use it. If you are in the US, go to this website to find a program near you that gives out naloxone rescue kits for free.

^ always good advice! Remember that Narcan is absolutely no substitute for calling an ambulance though - it has a very short half-life and is a holding measure only (this is especially important in heroin and other longer-acting opioid overdoses, but applies for any OD situation - the person is going to need medical assistance too..)

Try to avoid getting yourself into that position to start with though ey :)

Buccal absorption of fent isn't as easy to titrate as smoking and it can be quite variable - I would start with a really small piece first and see how you respond, I'd probably go with 1/8th of a 12.mcg/hr to be absolutely sure. As nAON says, give yourself plenty of time before trying a larger amount (12.5mcg/hr patch does have about 2mg of fent in yes, 2.063mg or something, can't remember precisely!)

Oral oxycodone to buccal fentanyl is a complicted and I would say very imprecise (given the variable bioavailability of each) calculation, so not really sensible to try to work it out - just start very low and procede very cautiously.

Some great advice, thanks so much.

In relation to naloxone, I actually tried really hard to get my hands on some here in Australia but I was treated really badly by hospital staff when I spoke to them about my unique condition "Autonomic Dysfunction" and my opiate sensitivity resulting in regular adverse reactions and how important it was for me to have a backup of naloxone which they refused to give me. Its ridicilious that I am having so much difficulty in obtaining some naloxone.

So sorry to hear about your experience- they hospital should be concerned about malpractice! Do you know Anex (Association for Prevention and Harm Reduction Programs Australia Inc.)? I think they are in Melbourne...I wonder if someone there could help you find a prescriber who is more reasonable? They also may be able to direct you to people who are distributing naloxone in a clandestine manner.
There is a naloxone investigation that is going through ethics review right now. The study would give naloxone to people with a history of heroin use coming out of prison. I imagine that it would be difficult to get yourself some of that narcan unless you enroll in the study (and are coming out of prison!), but what that does mean is that the take home naloxone profile will rise. (And there will be a black market for naloxone kits- I just heard of someone paying $75 for naloxone on the street!)

12,5 mcgr/h are the weakest patches, but fent is always fent...sometimes people overrated it, but of course it's better to be safe than sorry..you can always take more, you don't have to be impatient..nAON speaks very well, this is what you should do. Be safe dude and i hope the fuckin W/D leaves you fast..

Martin, thanks mate.

mds275, That is great news for me and some excellent advice. I will try the contacting Anex about it.

When the hospital treated me very rudely and escorted me out with a security guard all because I asked for naloxone, I reported them to the "Medical Practioners Board of Victoria". The investigations have been going on for about 8 months now but I am confident I will win this case. In their defense they made up a lot of excuses saying they didn't know anything about my medical history of "Autonomic Dysfunction" but that is all lame excuses. One of the doctors even denied yelling at me rudely and refusing me medical treatment during an adverse reaction by telling me to go elsewhere if I wanted help.

On that day I went into hopital I also took in a folder full of my medical history from various specialists all explaining how I have reactions to all substances that effects my Autonomic Nervous System, I even had statements from my Neurologist and GP saying that I am not a drug seeker but the hospital staff refused to read any of it and made judgements based on their own assumptions. Luckly the Medical Board has taken the matters very seriously and the hospital has now made a "care plan' for me consisting of a folder alerting the staff to my unique condition to prevent future problems

Altered Perception, is that the name you go by on here? (I know that is stupid question) this is my first time ever being in a forum. Anyway, I am very sorry you had to go through that with the Drs. It used to be the docs took care of things that have to do with medicine and let GOD take care of the judging but now days these bastards dont give a shit and anyone who in their mind is looking for drugs then the doc has his mind made up and your leg could be falling off but too bad your a "drug seeker" I'm sorry to hear about your experience man and hope that things are better now. what is naloxone? that is bullshit that they escorted you out for that shit. I hope you persue your case and win, best wishes to you.

tazwell, Altered Perception is indeed Altered Perception's username.

Naloxone (trade name: Narcan) is a drug used to reverse opioid/opiate overdoses. It is an antagonist at the opiate receptor (meaning it occupies the receptor but does not activate it). It is used by emergency medical personnel to reverse the acute effects of an opiate OD.

Altered Perception, good luck on aquiring some naloxone - it is completely ridiculous that you can't get hold of a drug that could potentially save your life. In the UK there are a few places offering training on use but I don't think it is widespread. I hope you manage to find someone in Aus who is prepared to prescribe you some! I am also so sorry you have been treated so badly by hospital doctors <3

This thread has gone off topic (although in a useful way!!) so I'm going to close it - AP, if you would like any more advice regarding fentanyl patches please pm me and I can reopen; alternatively, if anyone would like to start a thread with a query about naloxone/Narcan feel free :)

As always any queries feel free to shoot me a pm.
