Opioids Cross Tolerance

I don't really understand the idea of 'incomplete cross tolerance'. Is it supposed to mean that a tolerance for one opiate doesn't cross to another? Seems to me that wouldn't be true because when I was addicted to H back in the day any weaker opiate didn't really work, except in extremely high doses.

The reason I'm asking is because my base tolerance is 40mg of hydrocodone at one time. I've just come across some 30mg roxicodones and am trying to figure out the right starting dose. I know oxycodone is stronger than hydro but what I don't understand is if I have to start even smaller because it's a different kind of opiate. Will one 30mg roxi be safe?

I can't think of any specific examples right now, but incomplete cross tolerance is when the tolerance doesn't completely cross over to another drugs tolerance.
 
Cross-tolerance is when you have a tolerance to one drug (say hydrocodone) then you'll have tolerance to other similar drugs (like oxycodone). When cross-tolerance is incomplete it means that you won't have quite as much tolerance to the other drug. If you normally use 150mg of hydrocodone then you'll certainly have a tolerance to oxycodone and if that tolerance was complete, you'd require about 100mg of oxycodone. Since the cross-tolerance is incomplete, you can probably take a little less (some say as low as 25-50% of the dose, in my experience I find it more like 75% ).

Here is a good narcotic converter and it allows you to set your own percent reduction for incomplete cross tolerance.

A 30mg roxi will be safe but it might be more than you require/desire so you may want to start around 20mg... 30 is not going to kill you though but might be a little unpleasant if its too much.

The general consensus is that oxy is 1.5x as strong as hydrocodone mg:mg.

Hopefully that answers your question and in the future please use sites like the one above (that can be found in the directory) instead of starting a thread in here.
