Opioids "Cross-Tolerance Correction"

I've been using this Site for opioid conversions: http://www.globalrph.com/narcoticonv.htm However, there is something I don't understand -- the Site says to enter your "Reduction for Incomplete Cross-Tolerance", the usual reduction being between 25- 75% (apparently). I read the little help icon regarding this but I still don't understand it . I mean why would you reduce your "cross-tolerance"? And how do you know what percentage to reduce it by?

I would be grateful if someone could explain this "Cross-Tolerance Correction" feature.

all drugs affect people differently. It is impossible to know how a new drug will work in you, due to variations in metabolism, receptor binding, enzymes, etc... thus the only really safe way to get an equivalent opiate dose is to reduce by, on average, 50% when transitioning to a new opie.
