Opioids coming off oxy

been taking oxy 160mg x 2 daily and 2 20mg x 4+ daily for well over year and i'm now totally tolerant of it, no matter how much i take i get very little pain relief and basically no high 8) - gradually coming off it over the next few weeks then my doc is thinking about putting me on either hydromorphone or fentanyl. I know fentanyl is the strongest, but as a patch I doubt it would take as every patch i have put on my skin has always fallen off, be it nicotin patches, heat and cold patches etc. Would hydromorphone be a good step up?
i have 2 damaged bottom discs which are bulged and hitting my nerves etc. - waiting on surgery appointment! at pain clinic etc. etc.
now i also like to get high from it (pls dont start cause i'l just ignore ya), so which of the two mentioned would you prefer? lol

well with a oxy tolerance like you have got its gonna be tough i think maybe fentanyl for round clock pain relief but also a short acting pain killer for breakthrough pain like low oxy/hydromorphone as well hopefully the doctor will go along with that and then you can use the pills now n again for recreation but eventually with opiates the honeymoon is over and you use just to feel normal and keep anxety at bay

well with your given information I would go with the fent patches for chronic pain and maybe roxi 30s for breakthrough pain but seeing your tolerance to oral oxy, switching to dilaudid doesnt make sense because of BA unless you were going to be IVing your Dilaudid. ....personally I think either Fent/oxy or Fent/morphine would be the best choices

I'd be willing to guess he's already well aware of this. And snorting Oxycodone will make it hit harder, but I don't think it's technically more efficient. I'm pretty sure the BA is higher orally. So if anything I'd take off the wax-matrix and then eat them.

Furthermore, as a chronic pain patient these threads where people admit to abusing their Rx medicines really bother me.

There are lots of threads on fent and HM so you can easily use the search engine but let me tell you, every pain patient I've talked to on here who abused their scripts got fucked up the ass one way or another... Getting cut off, their tolerance growing too high and having to go to heroin, getting caught doc shopping etc.
