My statements regarding heartbeat are based on great quality cocaine found up until the late 80's. I could party on that cocaine for 3 days straight without even paying attention to my heartbeat. Sure, it was elevated, but not to the degree where it was a concern or noticable.
That may be from a subjective point of view, but it's a plain and simple fact that cocaine is cardiotoxic because of its combination of sympathetic stimulation and local anaesthetic activity. Cuts such as ephedrine or amphetamines may make it subjectively more noticable, but prolonged/heavy use of even pure cocaine will eventually lead to heart problems in a majority of users. For some it may be as trivial as a tightness in the chest due to cardiac insufficiency (which goes away after ceasing use), but for a lot it can progress to cardiomyopathy, tissue necrosis, heart block problems and possibly even ventricular fibrillation leading to myocardial infarction (a heart attack). If people were to do a lot of amphetamine mixed with lignocaine, they'd be predisposed to the same problems.
Cocaine's main cardiotoxicity comes not from its effects of sympathetic stimulation, but from it's local anaesthetic activity. Any local anaesthetic that becomes systemic (ie gets to the whole body, rather than just causing numbness in one part of the body) carries the risks outlined w.r.t. the heart. It does so by reducing the ability of heart muscle to contract and conduct messages co-ordinating contraction of the heart in the normal manner. The sympathetic stimulation is simply fuel to the fire as it makes the heart try to work harder (not good if the lack of co-ordination is reducing the supply of blood to all organs, including the heart)
