Can someone help me....PLEASE!!! (morphine/duragesic)

I am currently taking 320mg og exteded release morphine prescreibed by a PMD and have been switched to 100mcg Duragesic patches. My PDM says they are the same medicine. I know they are not....how do I safely taper one to another.

You don't need to taper the morphine. The conversion is just about right.
