Brothers leg looks bad due to IV Subutex HELP

I wasn't sure what forum to put this in. My apologies if this is the wrong one.
My brother has been injecting Subutex for several years. He has a VERY hard time finding a vein. He doesn't practice safe and clean injection practices and constantly saves and re-uses needles. His arm veins are pretty much un-hittable so he has been using his legs and extremities for a while.

About a week ago he started to develop a big, black mark that looks like a bruise on his leg where he shot up. There are red marks surrounding some of the bruise and tiny red bumps have started to raise.

Any idea of what it could be? Is this something he needs to worry about? I have included several links to some pictures of the area that I've uploaded:

Some pictures are better than others so look at a few, please. However, none of them really do the injury justice - it looks a lot worse than the pictures indicate. I'm really worried it could be something serious. Thanks for taking a look.
 
This advice is not intended to take the place of a phisicians. If he is worried about this then he should see a doctor.

Looks to me like just a bruise. I would take a permanent marker and trace around the red area that surounds it. If the read continues to exspand I would see a doc about a possible infection. If your brother is running a fever I would also see a doc. If the skin continues to deteriorate or change in ways not customary with a bruise then see a medical professional.

Looks to me like he pushed through the vein or damaged it in other ways and some blood leaked out. But its pretty hard for amateures to diagnose off pictures. A professional examination will be much more trustworthy.

Please ask your brother to show himself some love and his health more respect. Following clean and safe injection protocol is really easy and there really is no exscuse not to do it every time.

It might be an infection, but it's hard to tell by seeing the photos. He should get it checked, especially if he's feeling sick.

He won't go to the hospital. I tell him he NEEDS to go all the time. He shrugs it off. Sorry guys my typing is hard d and not coherent. Took 450mg lyrica for first time. Anyway check this out. His leg is worse:

It's bubbling up. Looks real bad. I'm worried about him. Tell me something about those pics. I need info sorry lyrica too strong . Please tell me something if u got the knowledge. Thanks and peace Be with us allllll
 
He needs to go to the hospital like right now. I know you care about him and dont want to make him mad but you need to force him to go. He may not die but if it is bad he will lose his leg. There are plenty of horror stories about people losing limbs because of IVing and no one is above this. You need to tell him he literally has 2 options go to the hospital now and get antibiotics or go to the hospital later and get them after they take the leg, either way hes going to end up going to the hospital going now just saves money.
 
Hes goin to wish he went to the hospital when hes walking around like a pirate with a peg leg..i will never understand risking a limb because of anxiety over the hospital..
 
I've clearly been beaten to it, but in the future, the pictures and details are unnecessary. If something looks "not right' - beyond mild swelling or irritation - only a doctor can give you a definitive diagnosis and treatment, not to mention peace of mind. People lose limbs and worse from IV complications all the time; it's not just some tall tale.
 
This has been going on for a few days now. We are not medical professionals and cannot say with any certainty what this is.
Take him to the hospital or call for an ambulance. No excuses, just get him there.
