Opioids Best Opiate painkillers for pain(long acting & breakthrough)

So I got into a huge car accident about a week ago. I have a big tolerance. The DR gave me fetanyl(25s) patches & Roxi 15's for the pain. Didnt work for crap. Called my dr Fri, but the office was closing. My bf who was in it w me got fetynal(50's) & 120 8mg dillaudids. He told me to basically try my bfs meds to see if they worked but basically I can tell him what I want. Im def going w thepatches but whats strongest for breakthrough pain. I was into the pill thing back in 2005 before Opanas came out. Are they really the best? If not whats better. Thanks.
