Benadryl - Diphenhydramine

This is an old thread. Still, it´s worth saying, Diphenhydramine (Benadryl) has a tendency to make Restless Legs/Akathisia much, much worse. If I were to take 300mg Diphenhydramine whilst in Opioid withdrawal, it would be a nightmare for me. I learned early on that Opioid withdrawal and Diphenhydramine do not mix for me and for that matter, most other people I know. Hydroxyzine (Vistaril) produces less RLS in my experience, so I would get that over Diphenhydramine if at all possible.
 
I quit opioids with just a few meds and antihistamine was one of them so i could share my experience on how to use them for making discomfort less uncomfortable.

1, Old antihistamines or low doses of some antipsychotics and mirtazapine can produce surprisingly intense sedation - they can be as sedating as minor tranqulisers ( benzodiazepines ) which are often used and approved for human consumption. Ofc there are some benzos with extremely potent hypnotic effect but these are less common and some of them are research chemicals.

If u want to withdrawal from opioids u need to adress this - nausea, diarrhea, insomnia, anxiety, pain.

Diphenhydramine is good for soft muscle relaxation and for nausea but it's not that good at making you sedated. Usually it works fine when used for the first time in 50 to 100mg dose to help you relax. But for some reason tolerance is almost instantly developed and higher doses are like taking brutal hallucinogen - complete delirium.
It also raise your serotonine ( slightly ) and this doesn't help for a lot of people, they actually need less of serotonine.

I would recommend PROMETHAZINE - 25mg for nausea, 50mg for general relaxation + potentiation of other sedatives and 100mg if u really suffer and need instant relief from both nausea, anxiety and discomfort.

But it definitelly works best when combined with other sedative.
I found that tizanidine ( muscle relaxation med ) 4mg every 4 hours with diazepam/alprazolam/lorazepam/clonazep. or any benzo in doses of equivalent to 30-40mg diazepam in single dose added to your antihistamine especially promethazine will produce fast and intense relief from every wd related struggle except diarrhea, depression and pain.

Diarrhea is easy to manage with loperamide.
Pain probably won't disappear completely for some time but tizanidine, benzos, promethazine will help but u also need to take otc pain medication. 1000mg paracetamol is actually good cns painkiller but something like diclofenac, aceclofenac, naproxen or ibuprofen are definitelly going to help more than only acetaminophen.
I also advice to use topical nsaid formulations.

So, this combination of meds will definitelly provide enormous relief because of this
- promethazine is not only antihistamine, it also decrease adrenaline, NMDA antagonism, very gentle dopamine antagonism - this will help with anxiety. And potent anticholinergic activity will help with gastrointestinal issues and prevent unvoluntary muscle movement.
- tizanidine will help with body temperature regulation, chills, muscle pain and it's also potent sedative but short acting
- benzodiazepines works for everything except nausea or serious insomnia or paranoia so using them will help and they are like 100% more potent if taken with promethazine.
- loperamide is your saviour because u can be sedated as much as possible but if u have to use toilet every 30 minutes than it's fucked up situation. So adding loperamide into this mix will provide relief from diarrhea so u can actually have some rest after using sedatives.
- otc painkillers - well, they are just useful for pain. Not healthy but for a few days they can really help. Especially combined with sedatives.
- optional = magnesium ( NMDA antagonism ), dextrometorfan ( another NMDA antagonism, little instant moodlift, higher doses are ultimate wd killers because high dose of dxm is definitelly potent psychoactive dissociative, painkiller, antidepressant and it also makes recovery faster but don't use syrup formulations and start with small doses like 60mg cause this is really underrated drug. This shit is like psychedelic morphine if u take heavy dose. Weed makes psychedelic part of it more intense.

And there are also antihistamines with special effects - chlirprothexine ( just 15-30mg and your body will be calm ). It's oldschool antipsychotic but it is used as needed for deep calming effects because it doesn't produce muscle tension like, i dont know haloperidol.

And if u want extremely potent and long acting antihistamine which actually raise your monoamine levels but still prevent nausea - mirtazapine. But no more than 30 mg.
