Not being able to shit can kill you. A fecal impaction or fecaloma can be caused by chronic constipation and after that happens, you can end up with an infection, septic shock, and necrotic tissue. The only way to remove a fecal impaction is either manually by a doctor under general anesthesia or surgically if it is severe enough.

Always eat enough fiber and drink plenty of fluids on opiates and opiods and if you experience a problem, nip it in the ass literally like Oblivion did. Don't wait for it to get worse.
 
Man- Methadone really is a bitch when it comes to constipation and IME laxatives really don't work all that well-the Best regards, advice I could give would be to keep oneself hydrated all the time and avoid anything that dehydrates you (like caffeine and antihistamines-diphenhydramine seems to make constipation with opiates much worse) unless you make sure to drink larger than average amounts of water every day.

I cannot stress how important it is to stay properly hydrated, while its not always immediately effective, it is the Best long term solution for constipation that I know, and it took me forever to put it in to practice.
 
Castor Oil. Simply the strongest laxative.

Lord fuck no 8o . A uncle of mine who was on morphine and dilaudid for severe chronic pain once got so constipated that he bought a bottle of that stuff at the pharmacy. It worked a little too good and he had diarrhea for days after even with the opiates and stomach cramps so bad that only high doses of buscopan and dilaudid relieved it. By the end of it he was so dehydrated that it took days of chugging Gatorade and water to get him hydrated again.

I find milk of magnesia to work the best for me personally with the least amount of stomach cramps.

QFT! While opiate withdrawal induced diarrhea is seriously annoying atleast staying hydrated will prevent anything bad from happening. Constipation is not only uncomfortable and sometimes down right painful but also life threatening. Avoid taking anti-histamines like diphenhydramine or any anti-cholinergics like tricyclics, anti-psychotics, etc if constipation is a problem for you on opiates as these drugs tend to make it much worse.

Zneg : I can vouch for PEG 3350 + lactulose + docusate, not too much docusate though because you will ex-pulse a lot of too soft shit if you know what I mean.

Most people never get to try PEG 3350, I had to mention it to the doctor and he gave me warnings about it : This ain't candy, if 250ml gives you diarrhea, drop the dose to 175ml or even 150ml.
 
If you're after IMMEDIATE 'expulsion', for me, glycerol suppositories have been a god send. I take a wedge of Venlafaxine for my depression which causes horrible constipation, like 8+ days. Every time I've popped 2 up the pipe works I literally cannot hold my cheeks together longer than 10 minutes. I get so much propulsion that my bum actually leaves the toilet seat. It's nasty and messy but it honestly works like a dream. I laugh about it now, but the pain that 8+days of constipation causes is just horrific.

So yeah, glycerol suppositories 8(
