It's not accurate to say it has to be a serotonergic drug as normally thought because tramadol plus gabapentin equals serotonin syndrome in some people. It is a very rare condition which arises from combination of more than one serotonergic substance or in even more rare cases one serotonergic one alone. I know there are some papers supposing that even non serotonergic substances like dexamphetamine could give serotonin syndrome but my hunch is that they didn't properly check for other co ingested and non admitted substances.
There shouldn't be any real risk from stuff like dexamph or methylphenidate. These both aren't serotonergic. Also I and many others did multiple serotonergic substances and didn't get any problems. I was even prescribed two SSRIs at a time and while I discontinued one it was cause to headache, not serotonin syndrome.
