Stimulants Adderall dosage (need guidance)

Hello friends, I am newly prescribed Adderall XR 20mg (once a day in the morning) and have been taking it as prescribed for 4 days now. Tolerance is quickly rising. Effects peak at late morning/noon and currently wear off late afternoon/early evening. Most likely will need a 30mg dosage. Adderall has helped me with anxiety and focus, as well as stabilize my mood with its euphoric effects and help me stay up late into the night when necessary.. with good sleep afterwards..so far.

Suggestions/advice in regards to dosage or IR vs. XR after the 1st pill wears off would be most appreciated. Thanks folks.

You may not actually be developing tolerance, the diminished sensation you're experiencing may just be your body becoming accustomed to the euphoric effects of Adderall (Dopamine transporter inhibition resulting in more increased Dopamine in the synapse due to decreased reuptake). Perhaps continue taking it for a couple weeks and really try to take note of weather it is truly not helping with your ADHD or if it is just your body adapting.

As far as IR vs. XR, in my personal experience, I find XR to be much more effective than IR.

That is a red flag. I was prescribed 15mg/day Dexedrine XR for 7 years and today, the 15mg I take feels comparable to how it felt in the first months. My body has absolutely no defense against it, it cuts through me like through butter. That's how it should be. Make sure you tell your doctor about the tolerance. You might need to switch to Clonidine/Memantine/Dexedrine.

SWIM prefers IR vs XR that way he can control my doses. For example if he would take 15mg IR right after breakfast 7-8am it would work great till around 3pm that's with food in the belly, we all know amphets XR have their full effect on empty stomach. But you won't have appetite for a while. If he ever had more stuff to do thru out the day till night he would only take half 15mg IR.
With XR it's tough to control your doses and when you open the capsule it's a mess. 20-30mg XR should hold for 8-10 hours without any problem unless just like mentioned before, you becoming depended on it "Building up tolerance". After long use of different doses than needed, you might feel like you need it more and next thing you can't sleep, anxiety kicks in, just a domino effect.
Doctors have code to follow, they prescribe things to people by the code. The part that's a downfall everyone reacts different to meds so if SWIMs weight is 225lb 30Mg XR is plenty for a day for him.

Also, keep in mind that Adderall, especially generic has a troubled history with ADHDers. Hundreds of reports claimed that Adderall:

- Suddenly stopped working after getting a refill to the point of being comparable to placebo. Investigations into brand/generic were inconclusive.
- Was inconsistent and unreliable, randomly stops working on days when you need it most and overstimulates you when you don't really need it.
- While levo-amphetamine provides a kick to most, some become angry and physically violent, leading to their arrest and revoking of their prescription.

1. Don't SWIM.
2. Weight isn't a factor in determining proper amphetamine dosage.
3. Opening an XR capsule allows you to accurately dose with 0.1mg precision. It is not a mess. You don't know how to dose.

I do know how to dose my amps because I had experience with it and as I stated in my comment I prefer IR. So if I cut it in a half I know exactly how much I'm taking and how long I'm expecting it to last. With capsules you count the salts, if you have to take a 1/4 of it?
Reference to Mess-yes it is, unless you are prepared or have experience with it. Tilt it to one side, open most of the times clear side is bigger so you tilt it that way and open other UP.

If a 40lb kid is given 15mg XR, you should not take 5.5x15mg if you are 220lb right? Alternatively, if 30mg XR works for you, you cannot do 40lb/220lb x 30mg = 5.45mg XR to treat a 40lb kid, it will have no effect.

I always looked at if you weight more it would have less effect because of the body mass and metabolism,but in science way you are correct. And thank you for clarifying the way of looking at dose of amps.
I'm not prescribed to amps and don't use them daily at all, I used them to tamper my WDs from opioids. 46 days clean since last use. I did do extensive research before I attempted my cold turkey, I guess I never looked at it from math prospective like you have explained..

I got a few in hopes to ween myself from oxy. It helps greatly with that but it makes me all twacked out. I can't stand it. I would do 15-20 mg and by the end of the day I would be paranoid and felt exactly like a meth comedown.

Does that happen to you? Or is it just me?

I posted my process info there, I had a 5 day schedule set up and when I felt tweaked I took SMALL dose of Benz .5mg even helped with sleep and stay a sleep. It worked for me, but can't say that for others out there. Sometimes people just change their drug of choice after that process.

If SWIY is looking into using adderall recreationally, SWIM would recommend 40mg XR for SWIY and see how that make SWIY feel. Don't dose up again until about 30 minutes to an hour goes by.

OR SWIY could crush the beads within the capsule and repack the capsule with the powder. Why? Because half of the adderall beads are Instant-Release(so SWIM has heard) and the other half are delayed and release later. Crushing the beads causes the extended-release to be broken, so SWIY would be hit by all 20mg at once if SWIY took it this way.

When you break the beads does it cut the life expectancy by half or more?

I've done it before without issues, the powder has to be thoroughly crushed so that it doesn't form empty spots with the active ingredient concentrating in other spots. The reason people do this is to convert XR to IR without crushing the beads in the mouth, thus exposing the mouth/throat/esophagus to the chemicals. There's also the fact that amphetamines have a weird, numbing taste that can in some people cause nausea.

Liver enzymes can have more of an effect on dose than weight. I take IR dexedrine 10-30 mg (lower so I talk to people in social situations and not be so lazy that I don't clean my apartment and 30 when I need to study hard) and I can take it up to five times a day. It was a CYP enzyme in my liver. I am a small guy and my dose has never fluctuated a lot, I often take less than I am supposed to actually because it is so relaxing and without it I am hyper but after that enzyme test, the pharmacist understood. I have no heart issues with it, no rise in heartbeat. The only way I notice needing another dose is I feel like I want to eat myself out of house and home and that taste that makes food disgusting goes away. I don't think that dose is good for many people but the way it burns through my system apparently is like someone taking 40-50 mg a day according to that test. I take IR because I don't develop a tolerance to the appetite thing and long acting medications have a 4-5 hour crash that makes me rage... but the XR thing seems to be a hit with most people. I like being able to control the dose based on needs in the short term or so I can take less doses... the XR meds I had to take twice a day and skipping the 2nd dose would make me rage like no other.

I am really hyper so I have little patience for waiting for them to work and the brand that makes the white diamond shaped dexedrine tabs are a 10 minute wait and then BAM, you are thinking clearly and settled down. You can feel the front of your brain just seem like it is coming together... I have been taking this since I was 5 and I am 33 now...

I am 5'7" and 150 lbs... eat healthy and aside from occasional 420 and my Ativan Klonopin combo added to lamictal and max dose of dexedrine in the summer because I get seasonal manic moods, I work out and feel just fine... the benzos make me feel like I have bricks in me feet when I take them for a month so i never stay on them too long.
