does anyone know how drug metabolism is affected when you take 2 substrates at the same time (such as hydrocodone and atomoxetine for cyp2d6 enzyme for example). would the effects of one or both be stronger or weaker? also, say you take one on a daily basis, how would it affect the metabolism of other substrates taken on a less consistent basis? thanks!

Anyway, I'm not sure but I imagine it depends on how much of the enzyme is being used for metabolism. If there wasn't enough CYP2D6 to keep metabolizing you might have a build up of both substances, I imagine that unless for some reason one is more chemically attractive to the enzyme, then they would both be metabolized evenly. Looking at half-life might be a way to judge this?
As for taking one on a daily basis, if it was using up all your CYP2D6 enzymes with being metabolized I imagine that other drugs ingested may not be metabolized as fast.

I'm pretty sure that the CYP enzymes don't get saturated very easy and taking two substrates in reasonable amounts won't impair the metabolism much.

Hydrocodone gets metabolised to hydromorphone in the liver by CYP2D6, but it seems that the effects you get are mostly due to the hydrocodone itself rather than the hydromorphone. (yay for CYP2D6 PM's) So taking another CYP2D6 substrate shouldn't be a problem.

I am not positive about this but my understanding is that by taking two substrates you are increasing the total number of CYP 450 (XY) because you are taking two substances that require the same enzyme. Now whether there is a change in the drug in your system, I don't know. However, I think by using multiple drugs which require the same substrate you will induce more enzymes to metabolize said of both drugs.
I hope that makes sense and as I said I am not sure if that is correct but from what I understand that is how it works.
