What I Don't Get About Methadone

So I tried methadone for the first time last night around 6pm as reported on another thread. I was told that I would need to wait 24 hours before taking anymore. However, looking at online articles by physicians I've seen where chronic pain patients are treating with doses split up between 3-4 times a day.

I understand the long half-life and that dosing isn't "necessary" but once every 24 hours. But I've been told that taking another dose before the 24 hour mark will only prolong the current high from the original dose. I know a lot about opiates except for methadone. I just don't get why another dose before 24 hours wouldn't do anything. Is it simply that it won't add to any euphoria but will have increased sedative and analgesic effects? If anyone can further explain this I would really appreciate it. I hope what I'm asking here makes sense.

For the record, I took a total of 15mg at 6pm last night. I took another 10mg about an hour ago or so ago (roughly 18 hours after first dose).

When physicians prescibe methadone for pain purposes, 2x or 3x daily dosing is usually prescribed, because euphoria is not their number 1 priority. Analgesia is, so dosing with that frequency should keep the peak plasma levels at their maximum in order to get the most efficient, well rounded, 'round the clock pain relief many chronic pain patients need. The methadone would also be prescribed in conjunction with a short acting opiate, for break through pain. Such as dilaudid, or IR oxycodone..

For recreation, re-dosing within the 24 hour window won't necessarily have a negative impact on your health, but it is just the safest recommendation. Re-dosing earlier will not increase the euphoria you are after, but simply prolong the high and euphoria you initially achieved.

My recommendation of not re-dosing within 24 hours was simply to help you get the maximum amount of euphoria out of your pills, and help you get the most out of them and not 'waste' any per se. You could re-dose sooner, just in my long experience with daily methadone, it isn't until 24 hours or so later, that re-dosing my daily dose will yield any positive effects. Say if I took my 75mg at 6am like normal, then took another 75mg at 2pm, it won' get me any higher, just increase the sedation and likely make me sleep all day. Now if I took all 150mg at once, the next day at 6am, I would get much higher than the other scenario.

(Don't take this much, just using my high tolerance as an example)
