Opioids switch from 3-20mg oxycontin CR (OPs) to 3-15mg morphine ER question

habboz - I will give it a chance, but I definitely need to beat the time release.

thanks adub.

I guess ill try crushing up one.

Morphine has terrible oral bio-availability. OP, you may want to try plugging. Morphine is considerably more powerful with this ROA.

+ 1. Morphine's BA is about 10-25% orally, roughly 60-75% when plugged. IV is most effective of course, but I don't think you want to go there.

Your doctor cut you way back by prescribing the morphine as a substitute, although I
am sure he was not doing so intentionally. He was probably just being safe and conservative.

You may find that they work for your pain, but I got zero pain control from a similar dosage and my tolerance is about the same as yours.

I was very careful about dosing when I received my script too (cutting in thirds, etc.) but I found that I needed 3x the prescribed dosage to feel *any* pain relief.

My brain is not working very well this morning so I can't remember the conversion rate of Morphine ERs to oxycodone, but the morphine is definitely a step down in potency. What MadWolf said in his previous post is absolutely correct.

Ethex is a generic brand.

wow that's bad news. so i guess crushing one and eating it would be ok?? Ill give it a day and if it doesn't work ill call & ask for Ethex I guess.....

He should have given you at least 30mg Morphine ER's, and even then you still will get more relief from the oxycontin vs. the morphine. You get so much more out of the oxy, about 87% oral BA, whereas morphine is only 15-20%. You should look around at a few pharmacies for the ethex generic OC's.........they are exactly the same as the old OC's....

Oh and plugging means up your ass. you get an increased ba with this.

Well if I give this a day or two (I just really don't want to call in tomorrow, I feel like my doc is going to get pissed).... and they don't work really, should I ask for my old RX back?? I guess I'm asking if they are going to continue to make the old OCs or if they are gone for good??

I think you should give the morphine a chance....and if it doesnt work very well for you...go to your doctor(or call him/her) and say..."the morphine isnt really cutting it, is there anything else I could try?....maybe I should try the oxy again?"

I wouldnt simply say..."the morphine didnt work, put me back on the oxy"...try to give the doctor a chance to discuss other options that are available. For all you know, there is another medication your doc is willing to try that might end up working better than the morphine or the oxy!

Thanks James.... I'm just such a worry wart and stuck in my ways (routine). My issue is that I take my oxy in the am (usually very early, like 2am, 3am, 4am.... cause I can't sleep because of the pain). My issue is - I need a strong immediate dose in the morning (and I also have to drive an hour to work in the morning.... so I have to function, worried morphine won't have same effects, I've never taken it) so should I cut the dose up tomorrow morning? Or just down it? And should I do 1 or 2?? Suggestions?? And also will it be similar to the ox??

I would take the drug however the doctor prescribed it. And if it still isnt killing your pain, then maybe you should tell him that and see what you can do from there.

Are you trying to just kill your pain with these pills, or are you actually trying to get a little high off of them?
think real hard about the answer because even people who are taking medications given to them by a doctor can very easily slide into an addiction/dependency and sometimes its not easy for them to admit to it, or sometimes even realize it.

Because ideally......the painkillers you are given shouldnt take away every last bit of pain you have, you should still be experiencing some pain....not even normal people eith no health issues live there lives pain free. its natural for humans to live life constantly adjusting to varying levels of pain. And the addiction comes into play when a patient is taking enough meds to prevent them from feeling ANY pain, and then the patient comes to expect to feel that way, and when they dont....they automatically assume they need more meds and go running to there doctor saying that they are still in pain....when in fact, it could be a good sign that you are still having some pain. Its actually NECESSARY to still experience pain while on strong opiods for pain management, otherwise you will DEFINITELY become dependant upon them and eventually start showing drug seeking behavior. Your walking a think line so be careful.

I am taking them for legitimate pain. I have been in many, many accidents and also have other back problems aside from that. In the mornings I just need something to act fast so I am not up at 3am in pain, waiting for an hour or two for the pain to subside.... That is my major concern.
I definitely understand how people can spiral out of control with them. I understand why they are highly addictive as well. Am I out of control with them? No, I do not believe so. I only take how much is prescribed to me, never more. I never ask for fills early. I also do not take ANY other meds other than from my pain doc (which I REALLY need to be taking my anxiety meds, but I'm scared to mix, even tho pain doc said it's okay). I also do not drink. Am I addicted? Probably........ without them I could not function normally.... I would be in too much pain. In my case - I would much rather depend on meds to function as a normal member of society than to suffer in pain & be miserable because I could NEVER sleep with the pain, it was next to impossible, not to mention dealing with the pain itself 24/7.

I'm just super nervous about this whole situation. I hate change, I don't deal well with it. I just want to keep doing what was working for me .... I don't understand why this happened this way - why didn't he just write me a script for the Ethex specifically?? I don't get it....
I am dreading tomorrow morning.... do I take it whole and wait & wait & wait for possible relief? Or cut it and make it IR? I guess I'll give it a go whole.... if it doesn't work, I'll ask if he can write for Ethex.... that's if it's in my area.... UGH I hate hate hate hate this worry. But my fingers are crossed for this to work, it would be nice to be off the oxy. I just have my doubts because this is the only thing that's worked thus far and I almost feel like he was "punishing" me today for bringing back the OPs.... that's what bothers me most, their reaction to me today. I felt like SUCH a POS when all I wanted was what I was prescribed the month before, I didn't want MORE - I just wanted things the same .... and I feel like I got "punished" for that by being given the morphine. Idk, maybe I'm going to deep into this.... I'll just take it tomorrow morning & see what happens.... hope for the best. Any input is welcomed. Thanks.

You could try talking your doctor in to prescribing you IR OC instead of ER OC or morphine. Maybe 10mg's every 4 hours. It may be tough getting him to prescribe instant release OC instead of ER, its possible he could get suspicious, but with your options it seems worth a shot. I guess it depends on how cool your doctor is. I highly doubt you will enjoy ER morphine unless you are shooting it, they just suck any other ROA IMO.

Good Luck.

Since it was not specified for this OP that otherwise seems to have no knowledge of the issue of plugging...OP, if you try the plugging ROA, just be aware you need to dissolove the pill in a tiny amount of water and then apply it up the poop shoot using something like a syringe(without the needle of course). Push the delivery device in so that you at least get 4" inserted. The solution must be injected deep enough that it does not leak. Use some type of lubricant on the syringe for greater comfort.

Ok so here's an update -
I took 2 this morning (cut, because I didn't want to take a whole at first.....) I took the first 1/2 at 4:30 am and by 530 I had taken 2 wholes. Still nothing. Then I decided at 6am - why sit around? So I got up and started my drive to work (plus I had missed a bunch of time this week, picking up my first RX, then going to the pharm, then going BACK to the doc, back to the pharm, etc. etc.) .... so as I was driving along and kind of suddenly started to feel better, the pain was subsiding - if you had asked me moments before I left the house, I would have told you these things did NOTHING for me and my back hurt more and more with every movement, I was deciding when to call my doctor to ask to go back to the old meds.... but suddenly on the drive things were getting better.. it was very very very very weird... so I got to work around 630-7am and the pain was pretty low, which was wonderful. THEN around 830 the pain started coming back and by 930-10 I was in full blown pain again, awful. I took my percocet around lunch and that barely did ANYTHING. I don't know if the combo just doesn't work well with one another or what.... am I crazy here??
So now - I just took my last dose of perc and I don't feel like I've taken anything. I'm sitting here in a terrible terrible mood and in pain, pissed & confused .... Idk what to do.
So I'll give it tonight - see what happens..... but I'm miserable. Do you think it'd be bad to call my doc and ask him to put me back on my old meds?? I know you guys are not doctors, but I'm just looking for an outside opinion here. I don't need him to think I'm "seeking" (Which I am REALLY not, honestly) I just need to function normally. My back KILLS me when I drive and especially at night - and with the seasons changing here it's just getting worse.... ugh. I feel so alone and helpless and feel punished by my doc because my body was not agreeing with those OPs.... IJDK.
Opinions please on what I should do....

I have no doubt that you are in legitimate pain....what I was saying that sometimes doctors prescribe too many opiates, or the patients ask for more opiates, because they are still experiencing some pain. And my point was that you are SUPPOSE to feel some pain. That is the way humans live there lives naturally. We dont walk around in a world of bliss, never having to sit down because our back hurts, or never having to take some tylenol because we jsut generally feel achy overall.
If you are given so many narcotic pain medications(or soo high doses) that you are experiencing no pain practically whatsoever....you begin to feel like this is how you were MEANT to feel....when in fact, the best way for a pain management doctor to prescribe medications, is to give you just enough so that you still have some pain but you are still able to live your life and function for the most part like an average human being. Very few people take the time to think about this concept and realize that...'hey!...my medications COMPLETELY take away any pain that im having, and that is how I have come to expect to live.....and whenever I even feel a hint of pain, i go to my doctor so he can increase me dose.".....these problems are not only caused by the patient wanting to feel a "high" or just maybe "pleasure and no pain" all the time....these problems are also a result of the doctors bad decisions. Lots of times, a doctors patient will become accustomed to being given mountains of opiates so they never have to experience pain again.....but this will DEFINITELY lead to addiction. You need to feel some pain in order to keep your brain in check and prevent you from showing drug seeking behavior everytme your back or feet hurt.

Do you understand the point im trying to get across here?

I think I understand what you are saying.

The point I am at right now is miserable. This new med doesn't work much at all and I am in so much pain it's preventing me from being normal, not to mention I'm crankier and snippier than ever, so I apologize if any of this comes off as nasty, I'm honestly not trying to be. Anyways - I have given the meds 2.5 days so far, plus today.... it's awful. they take 2-3 hours to kick in and do not last. I really think I am going to call my doctor on Monday because my previous meds were much more effective in combating my pain. I just don't want to piss him off, but if it comes to pissing him off vs me staying in unbearable pain, I'll choose the first option. What is the worst he could do if I call on Monday and request to be switched back to my previous medications?? I mean requesting to go back to previous meds isn't illegal or anything right??
Thank you everyone for your input....

I believe that you are in serious pain that needs to be taken care of and that the medication your doctor has you on right now is not working as well as it should be.

But I also think that you shouldnt necessarily ask to go back to your last meds. Maybe ask if he could give you "somethign else" for the pain. That may end up being your last med, but it may be somethign new and better than anythign you have tried yet. your limiting yourself to only 2 types of medication when there is a whole world full of different types of pain meds.

Also....keeep in mind what I said about still being in a small amount of pain to prevent addiction. I believe that is monumentally important.
