Drug users are well informed about the harms associated with the drugs they use, and perceive alcohol and tobacco to be amongst the most dangerous substances, according to a survey by UCL (University College London) and Imperial College London researchers. The findings, published in the Journal of Psychopharmacology, suggest that the current system of classifying psychoactive drugs in the UK may need to be revisited.

The study, led by Dr Celia Morgan and Professor Valerie Curran at UCL, surveyed 1,500 UK drug users via the website www.nationaldrugsurvey.org. Drug users were asked to rate twenty psychoactive substances on a 'rational' scale previously developed by Professor David Nutt, Imperial College London, who collaborated on this study. Heroin, crack and cocaine topped the list in terms of harm, but alcohol was rated fifth, solvents seventh and tobacco ninth. Ecstasy came 13th in the harm rating, LSD 16th and cannabis 18th. Thus, the survey found no relationship between the drug's legal status, based on the current classification system, and users' ratings of harm. In the UK, the Misuse of Drugs Act (1971) currently classifies psychoactive drugs as A, B or C, though alcohol and tobacco remain unclassified.

Dr Celia Morgan, UCL Clinical Psychopharmacology Unit, says: "Given that the Misuse of Drugs Act aims to signal to young people the harmfulness of drugs, this suggests a flaw with the current classification of drugs. We found that drug users rated legal substances such as alcohol and tobacco as more harmful than Class A substances like LSD and ecstasy. We found a high correlation between harm ratings by users and those made previously by scientific experts across all substances, suggesting users are well informed about the harms of drugs.

"The reported prevalence of use of each substance also suggests that the classification of drugs has little bearing on the choice of whether to use substances or not. For example ecstasy, a Class A substance, was the fourth most regularly used psychoactive drug, according to our survey.

"We also asked drug users about their perceived benefits of taking psychoactive substances, as this is clearly important in a person's decision of whether to take a drug or not. Psychoactive substances LSD, cannabis and ecstasy were consistently rated as having the highest short and long-term benefits. These findings add to the debate on the validity of the current classification of drugs in the UK.

"Worldwide, there are an estimated two billion alcohol users, 1.3 billion smokers and 185 million users of other drugs. Despite public health campaigns, levels of substance misuse continue to rise. One of the reasons for this may be the public's confusion about the actual risks of different drugs as they often receive conflicting messages from the legal system, the media and health campaigns. We recommend that future health campaigns consider whether to include the benefits of some drugs. By only citing harms, such campaigns likely represent - from a user's perspective - an unbalanced view and may mean that the overall message is more likely to be ignored."

The authors are following up the study with the launch of a new larger survey, in collaboration with the Beckley Foundation, hosted at internationaldrugsurvey.org.

The 20 substances surveyed in the 2009 study were alcohol, alkyl nitrates, amphetamines, anabolic steroids, barbiturates, benzodiazepines, buprenorphine, cannabis, cocaine, crack, ecstasy, GHB, heroin, LSD, ketamine, khat, 4-MTA, methylphenidate, solvents, street methadone and tobacco.

Participants were asked to rate them according to physical (acute or chronic) harm, psychological or physical dependence, intensity of pleasure, intoxication and social effects, including costs to the health service.

A similar survey of experts including psychiatrists and pharmacologists led by Professor Nutt in 2007 found that, of the same 20 psychoactive substances rated on a 'rational' scale, experts rated alcohol as the fifth most dangerous drug, whereas MDMA/ecstasy was rated 18th out of twenty, despite its Class A status. Overall, there was no relationship between a drug's legal status and its rated harmfulness. The 2007 study was published in the Lancet.

i guess this kind of shit is quite surprising to someone who thinks drug users are as ignorant about the shit they ingest as the average smoker

This is good :) I like seeing articles like this. As Thujone said, most people are under the assumption that "all druggies are dumb as shit" when they can be some of the smartest people around.

I think one of the reasons people choose to use "illegal" drugs is because they ARE well informed about the dangers of substances. Glad this survey shows that the "legal" drugs are just as dangerous and potentially harmful as the so-called recreational ones.

^ Exactly Chic ;) There's been so much more research put into them and, unless you have no problem with potentially being seriously hurt or die, you'd choose the drugs that the dangers and harm reduction techniques are known about.

Well, at least we get some credit! We're not all a bunch of dumb teenagers who raid mummy's medicine cabinets trying to get high from everything within reach.

^ Ok, some of us might be! But I can proudly say I never stooped that low.

Don't forget the respondents are people who use the internet. It was an internet survey.

Generally the kind of people who would reply to a survey like that already have a good grasp of the truth on drugs.

Believe me IRL around here, drug users are dumb as shit. I often get the "cocaine and heroin come from natural sources so they're better for you than that acid you're taking" :| These are the people who don't know how to do simple google searches and have no desire to read and learn.

Bluelighters are full of win though ;)

Pretty much all my friends use some substance and a lot of them are smarter than your average upstanding citizen. I like to think I'm a "sleeper", in that I don't convey any outward "drug user" image or behavior. Then when people get to know me they find out I do "druuuuugsz" and end up shitting themselves because I'm not balled up in a gutter with needles sticking out of me. That's how you breakdown stereotypes, one person at a time.

haha of course we can't all be winners. The thing that annoys me the most is the drug users that are dumb as shit and try to come off acting like they know what they are talking about. Often times they are some of the most misinformed, urban legend spreading assholes I've ever met. DUUUUDE TEH ACIDSZ STAYSZ IN YOUR SPINE FOREVER AND MAKES YOUR BRAIN BLEEEEED AND SHIT. I DID 1 HIT AND I WAS TRIPPING FOR 4 DAYS SEEING ELVES AND PINK ELEPHANTSZ MANNNGG.

That's exactly what we have around here. Those are the people who are insistent that cocaine and heroin are better for me than LSD. :|

That's really a damn shame on multiple levels. LSD has done nothing but benefit me. Cocaine gave me some of the worst months of my life.

^ sounds like the same bunch that's convinced Ecstasy is a combination of coke, heroin, lsd and goof balls

Perhaps to a degree. I think most prefer illicit instead of legal because the legal options suck balls.

Yes. IME, some of which comes from volunteering at a local needle exchange, the average user gets their knowledge from word of mouth, basically drug folklore and urban legends. Hell, most people don't even know how dangerous alcohol is, let alone illicit drugs that have been affected by propaganda for decades.

FINALLY someone said it, and said it RIGHT!!! D.A.R.E. lied about marijuana, and after I had tried it my first time and realized it was harmless, the FIRST thing that crossed my mind, seriously FIRST thing, was "What ELSE did they lie about????"

Gateway theory??? ONLY applies because of the fact that it's illegal!!! The only reason pot got me interested in other drugs was because I realized one was perfectly safe and PLENTY of fun. So I was ready to find more just like it.

This is education we're talking about. If they wanted to keep me safe from drugs so badly, maybe they shouldn't have taught me bullshit.

^pot isnt harmLESS, its just not very harmFUL...just funFULL

Im glad someone has said this. I just wonder how many articles in GB and America are going to say something about the drugs laws being "considered" for an adjustment or redone entirely before something actually happens to them.

It is nice to see a media source recognising not all drug users are dumb and also that the legal status of a drug often does not directly relate to the potential harm it can cause.

I believe this to be true about most bluelighters...they know their shit and at least aknowledge when they have to do something they know is dumb (think re-using old rigs)

This cannot be said of probably about half of the ppl I know that do drugs. When I tell all the kids I see doing adderal and being in college I see it a lot none of them believe me that its just amphetamines...basically.And trying to explain BA to most people I fucks with they think I'm retarded. They just can't comprehend bioavailability. A lot of things like this bother me terribly but I know that it's just my surroundings. Most avid drug users know what they are doing
