Benzos Diazepam for severe opioid discontinuation.

Hello friends.
As many of u know I am long-term heavy opioid and benzodiazepine addict.
I made the decission of really quitting by choosing one opioid - dihydrocoide to taper quite rapidly and i also quit 10mg alprazolam daily.
My psych prescribed me in my opinion very subtle and the most expensive benzo in my country - 10mg diazepam pills.
Usually i could take 200mg diazepam and feel less anxiety, comparable to 7-8mg alprazolam.
But now, in this ugly situation of fast taper and also switch from superior benzo to low-potency one i realised that even after big DHC reduction and no more alprazolam, this diazepam calms me much more than ever before. Like it cover the absence of alprazolam and also reduce my body temperature, muscle pain. But for Insomnia and panic attacts it's like placebo pill. Strange.
But i was also prescribed promethazine 25mg/1pill , take 1-2 as needed for vomiting and to help me sleep. This old antihistamine with anticholinergic and D2 mild antagonistic effects somehow Made low doses of DHC + low doses of diazepam so much more tolerable ( for beginner it would be potentiation ).
I don't understand why this one, promethazine, made my symptoms noticably less severe while my mirtazapine 15mg - also very strong antihistamine just not only doesn't calm me down. It even boosts my energy after waking up in the morning.
Only other meds i take are vortioxetine which Is my savior in treating depression, low mood, cognitive fog and loperamide low dose ( 4mg once daily ), metamizole ( good for pain ), caffeine and now also little bit of very pure ice ( it always helped me for wds and it makes me able to take care of my baby cat, my dog, basically it wakes me up. Won't use longer than for next 2-3 days. Because it Is like borrowing energy which u don't have but soon it want all it gave to return

So, why Is diazepam and also dihydrocodeine suddenly much, different or more noticable now ? Is it because of promethazine and if yes, why ?

Diazepam (Valium) 10-50mg day is the gold standard . Best Benzo easily

Avizafone (Pro-Diazepam) is even better, rapid acting water soluable pro-drug IM injection taking effects in half the time and reaching higher levels in CNS

Diclazepam (2-Chloro Diazepam) is my personal favourite……200h + half life and active in body 7 days approximately from a single dose ……3 active benzodiazepine metabolites

Diclazepam >Delorazepam>>Lorazpam (Ativan) ……has nearly 2,000mg
1mg standard ….I took 3mg usual days amd days and days of effective anxiolytic
 
However, for opioid discontinuation the gold standard and most effective staple compound is Clonidine 0.1mg tablets

It lowers Norepinephrine in the CNS which reduces BP & HR, eliminating sweating, restlessness, anxiety, aches in legs, hot & cold flashes, etc……a VERY essential drug

I cold turkey stopped heavy opioid use 20 years ago, hi dose Oxycodone, IV Dilaudid & H, and just stopped…..2 weeks of hell

1-3 days are miserable and you’re a mess
3-7/10 days are HORRIBLE, you’re non functional at home curled up in a ball contemplating suicidal thoughts
10-14 days withdrawal effects gradually subsiding

2 weeks after stopping short acting opioids and physically you’re ok, BUT mentally you’re still thinking of scoring opioids but you’re functional

Clonidine 0.1mg will make those symptoms 1/4 the intensity
Diazepam (Valium) 10mg tablets would be an amazing anxiolytic & muscle relaxant
Bupropion (Wellbutrin) 150-300mg XR Dopamine & NET reuptake inhibitor will increase energy & boost mood
Tramadol (low dose) a weak atypical opioid 1/10 morphine strength makes withdrawal lighter and also acts as a NET & SERT reuptake inhibitor, mood-boosting properties, happier ….but low dose temporary 1 week

Various off-label uses several compounds have…..which doc will prescribe ?

I am detoxing now. Diazepam was surprisingly helpful but now i only have alprazolam to help me detoxing and, yes Its much more potent but in detox it doesnt help me like diazepam. Much less, dont really understand why. Check my HUGE RECOVERY thread

I could not get clonidine, but tizanidine is great help now since i am on detox for 7 days now.

Alprazolam (Xanax) I was going to mention too…..a very rapid acting potent anxiolytic with antidepressant & mood-boosting properties as it releasing more dopamine as compared to other benzodiazepines

Sleeping is always an issue …..a good hypnotic for bed is needed. Clonidine fits that bill, but what they gave you probably has the same benefits

You’ll be fine soon enough. All the best. It truly is a horrific experience and I was amazed when I finally felt better 2 weeks after, how insanely horrific it was to opioid withdrawal. Essential medication is absolutely necessary and amazingly helpful

I also completely quit mirtazapine, vortioxetine, dihydrocodeine, cigarettes...Everything. Only tramadol, benzos are left to somehow taper down.
I have low dose chlorprothixen pills which are great for sleep but i am not sure about using them.
 
Low dose Tramadol is a useful alternative to slowly taper as its only 1/10th potency of oral Morphine and in addition increases Serotonin & Norepinephrine in the CNS , so you’ll be happier with more energy . My tablets were scored so can be broken in half…..or crush up into powder and take 1/4 or 1/3 of tablet at a time and decrease

Mornings are essential…..after lunch / dinner (light dose)…..and an hour prior to bed for an uninterrupted restful sleep

Slowly reduce daily by tiny amount ……especially for work so you can function properly these meds are essential

Having a good doctor and or access to these essential controlled medications is paramount

Sounds like you’re doing it right though
