Heroin what to do if someone is OVER DOSING and no narcan available.

a friend has been using heroin for a couple of months now and a few days he shot up (IV) and went unconscious. he wouldn't wake up no matter how hard i tried but was breathing fine. anyways he eventually came up to it but scared the shit out of me.

i live in Ethiopia where there is no narcan and calling an ambulance is the absolute last thing u wanna do + they wouldn't respond quick either.

is there an alternative to narcan? a drug perhaps with another intended use i might find at a pharmacy.

also what would happen if i gave someone over dosing Meth IV?

if no narcane is available you need to man up and call 911 and put the phone on speaker and set it on the floor.
then follow these steps:

  1. Lay the victim down on the floor. Before placing them on their back, visibly and PHYSICALLY (with your fingers) check the mouth for vomit.
  2. If there is no vomit go to step 3. If vomit is detected prepare for a work out as it will likely continue and you will need to lift from under the shoulders and pull their head towards your chest. Tilt their head sideways if it isn't doing so naturally and don't let their head tilt back. Allow the vomiting to complete (it will likely return) and IMMEDIATELY place them flat on the floor for step 3.
  3. Lay them on the floor on their back, remove your shirt or grab a pillow if one is within reach (every [I REPEAT] EVERY SECOND COUNTS. Place the soft object under their neck so you can tilt their head back.
  4. Check if they are breathing or have laboured breathing. You should already be talking to a 911 operator and have given them your address. If they are not breathing at all, go to step 6.
  5. If they have laboured breathing ensure there is nothing foreign or liquid in their mouth and throat area. Observe their nostrils for vomit. CLEAR ALL OBSTRUCTIONS.
  6. Close their nose with your thumb and index finger with their head tilted back. Take as deep a breath you can and blow it into their mouth. Your mouth should be tight enough to theirs that you can audibly hear the air entering their airways and visibly see their chest raise slightly. Do this at least twice and no more than 4 times.
  7. Think of the song STAYING ALIVE BY THE BEEGEE'S. This is the correct number of compressions you need to give to the chest. You should be pressing in a spot where their heart can be or would be felt by either your finger tips or palm depending which side of their body you're on. [ST]aying [AL]ive [ST]aying [AL]ive, [OH] [OH] [OH] [ST]aying alive. You want to press on the [ST] & [AL] & [OH] parts in my previous sentence. Another way is quickly thinking in your head "ONE MISSISSIP-" and repeating that exactly in your head. You should be on your 10th[MIN]-11th compression by second 6.
  8. After every 25 compressions you need to re-check their mouth for obstructions and if your fingers that pinch the nose are moist. REPEAR STEP 6 and do three deep breaths of air into their lungs. You need to be quick, you don't want to hold this air in so all the oxygen is absorbed by your lungs air sac's. As soon as you can no longer hold more air BLOW INTO THEIR MOUTH WHILE CONTINUING TO PINCH THEIR NOSE. Remember you should see their chest raise for every breath. You need to be so tight on their mouth that no air can escape whatsoever (including through their nose). Remember - this person is literally dying in your arms and you are, until paramedics arrive, the only one that can save their life!!!!!!!!!!!!!!!!
  9. REPEAT STEP 7 AND 6 and if you can hear them breathing adequate oxygen (not short, teeny breaths but breaths that last a minimum of 1.10-1.25 seconds minimum stop compressions and only continue step 6. STEP 7 IS EXPLAINED FURTHER BELOW.
  10. ROLL THEM ON THEIR SIDE INTO THE RECOVERY POSITION WITH THEIR ARM CLOSEST TO THE FLOOR BENT WITH THEIR HAND NEAR THE FACE. USE THE OTHER ARM AND PLACE IT ON THEIR SIDE OR BEHIND THEIR BACK IF THERE ARE NO PILLOWS. IF THERE ARE PILLOWS PLACE THEM TIGHTLY BEHIND THEIR BACK. OBSERVE THEM UNTIL PARAMEDICS ARRIVE TO ENSURE THEY DO NOT VOMIT OR ROLL OVER. THEIR FACE NEEDS TO BE TILTED TOWARD THE FLOOR SO ANY VOMIT CAN FLOW OUT OF THEIR MOUTH,
  11. OBSERVE THEIR BREATHING AUDIBLY AND METICULOUSLY. PUT YOUR EAR TO THEIR MOUTH AND NOSE AREA. LISTEN CAREFULLY. ENSURE THEY ARE BREATHING.
  12. WHEN ALL IS SAID AND DONE: ACQUIRE A NALOXONE KIT IMMEDIATELY. I CAN SEND YOU SOME IN THE MAIL. IF YOU'D LIKE HELP WITH HOW TO ADMINISTER IT I CAN EXPLAIN THIS TO YOU IF YOU REQUEST. BUT REMEMBER, IT IS IM. USE THEIR ASS FAT OR THEIR UPPER ARM OR BOTTOM OF UPPER THIGH AND NALOXONE NEEDS ARE LARGE THICK GAUGES. DEPENDING ON THE SEVERITY OF THE OVERDOSE AND WHEN THE VICTIM WAS FOUND (if an unknown time of them unconscious is discovered) STAB THROUGH THEIR JEANS INTO THEIR ASS-FAT. INJECT AT A 90 DEGREE ANGLE AND GO SLOW-FAST. NOT TO FAST - DON'T JAB IT IN LIKE IT'S PULP FICTION BUT FAST ENOUGH IT WILL PIERCE THEIR CLOTHING. ADMINISTER 3X FASTER THAN YOU WOULD INJECT IV.
    • STEP 7 REQUIRES STRENGTH. YOU WILL BE BREAKING AT LEAST ONE RIB IF THEY HAVEN'T BEGAN BREATHING WITHIN A FEW SECONDS. WHEN MY BROTHER DIED, I BROKE 3 RIBS IN 52 MINUTES AND AT 10 MINUTES I WAS SO TIRED I COULD BARELY GO ON. BUT I BROKE HIS RIB/ANOTHER RIB FOR THE THIRD LOUD "CRACK!" WHEN THE PARAMEDICS ARRIVED AT OUR FRONT DOOR.
    • CHEST COMPRESSIONS REQUIRE REAL STRENGTH. EVERY COMPRESSION SHOULD CAUSE A AUDIBLE EXITING AND ENTERING OF AIR TO THE LUNGS. IF NOT GO LOWER WHILE MAINTAINING YOUR HANDS ON THE SOLAR PLEXUS. DON'T GO LOWER THEN THEIR PECK MUSCLES OR BREASTS/BREAST AREA. If you do not hear air exiting on the push and entering on the release you're not pressing hard enough SO PRESS HARDER A LIFE IS LITERALLY IN YOUR CONTROL.
    • If they are vomiting through this process do what I did when a friend of mine OD'd on fentanyl. Lift their head from under the armpits and tilt their chest and head to the side. Vomit will be coming out of their mouth and even their nose. It's imperative YOU clear this vomit with your fingers EVERY TIME as FAST as you can. EVERY SECOND COUNTS. LITERALLY. EVERY. SECOND.
do not be a bitch. CALL 9-1-1. Accept the concequences and tell your friend they owe you if you are charged with possession. Most states in the US have laws that protect you from prosecution in these cases HOWEVER most judges will also let you off with a warning and at MOST a small fine because you did what was right. You saved a life.

CPR for 52 miinutes straight. I believe I fainted as I don't have recollection of paramedics taking over, merely them entering the house. Like i said, at minute 10 I could hardly go on. and when my friend who was vomiting OD'd I ran out of stamina after maybe 15 minutes fortunately his brother came home and took over and I explained through my laboured breathing what to do as I ripped my shirt off due to it being glued to me with sweat. But my brother? That was my dad, my brother and my best friend and I will forever be broken without him here </3
Who knows maybe a moderator will sticky this as it is an important question.

I also got a brother (older) who's basically like my best friend. Both of us got our own addictions but his main one is opiate addiction (fent) and reading that struck me really hard and even tear up. I worry about him constantly if he's okay or hurt out there no way knowing until it's too late, and gripe with meth here and there just to numb that part of my brain with that anxiety and emotions temporarily. I feel for you bro and sorry for your loss honestly bro. I hope you're doing good for yourself now man and keeping your head up, nobody should ever bear this horrible type of pain.

thank you very much for this incredibly detailed...

I'm sorry for your loss!

Dial 911 and put your phone on speaker. Begin performing CPR immediately. The chest compressions are more important than the breaths. If you can do both though it is a better outcome. In the most simple terms, use the heel of your hand to press directly on their sternum while your other hand wraps around it for extra support. You make compressions to the tempo of stayin' alive by the Bee Gees. You hold their nose, tip their head back and breathe into them about 3 times per minute.

You have to keep performing CPR until the paramedics get there. You will potentially get tired. It's a good idea to yell for help.

Remember, Opioids are deadly as they cause our nervous system to "forget" to breathe. So, theoretically, a person can keep a person alive, possible indefinitely, by performing proper CPR with breaths. In practice, nobody ever even tries CPR and it sucks. You watch people crying and shaking their loved ones when they should be performing CPR which would more than likely save their lives.

CPR is not difficult. By knowing it, you can be the difference in a really hairy situation.

Moving on. Injecting someone with Methamphetamine to reverse an overdose? Possibly, though it's not something that I would ever recommend under any circumstances. That's a good way to go from an immune "Good Samaritan" to premeditated murder by poisoning.
