I've never heard of it before, but a Google search tells me that Xtampza is extended release oxycodone. That might explain why you feel it's a bit speedy compared to morphine. It's a bit weird that you'd be on two separate courses of oxycodone. Oxy is stronger than morphine when taken orally. What is the dosage of your Xtampza? Are you able to request going back onto the morphine?
It is NOT a bit weird being on "two seperate courses of Oxycodone" when one is an instant release formulation(Percocet aka Oxycodone/APAP) and the other is an extended release formulation(Xtampza)!
In fact its EXTREMELY common to be on an extended release opioid for "around-the-clock" pain management and an instant release opioid taken as needed for breakthrough pain.
Because anyone who has had the misfortune to have to be prescribed and ER opioid knows they never last the whole amount of time their supposed to.
Or sometimes they're not as effective due to some physiological and/or neurochemical reason or something happened to cause a large increase in your pain.
In all of those cases a person is experiencing breakthrough pain which is when/why they would take their AS NEEDED IR opioid
Take me for example I'm currently prescribed two 27mg Xtampza every 12hrs and 10mg Oxycodone IR 4-6x a day as needed for breakthrough pain.
I take two 27mg Xtampza every 12hrs because prior to starting Xtampza before my insurance stopped covering Oxycontin I was taking 60mg Oxycontin every 12hrs...
36mg is the highest dose Xtampza is available in. Below are the other doses Xtampza its available in along with the doses of Oxycontin they're equivalent to...
•9mg equivalent to10mg Oxycontin
•13.5mg equivalent to 15mg Oxycontin
•18mg equivalent to 20mg Oxycontin
•27mg equivalent to 30mg Oxycontin
•36mg equivalent to 40mg Oxycontin
The Xtampza manufacturers website doesn't explain anywhere why it doesn't come in a dose higher than 36mg. But it does explain how in order to take a dose equivalent to say 80mg of Oxycontin a person much take Two 36mg Xtampza at once.
In fact in the downloadable Prescribing Information PDF goes even further to explain that since the maximum daily dose of Oxycontin the FDA allows to be prescribed to a single patient is 320mg. Since 320mg of Oxycontin is equivalent to 288mg of Xtampza that means a single patient is allowed to be prescribed eight 36mg Xtampza a day taking four capsules every 12hrs!

The formation of Myristate salt is achieved by combining a fatty acid called Myristic Acid and a base.
Which in the instance of Xtampza, the base used is Oxycodone Base(Oxy Free Base to be precise).
So the formation of Oxycodone Myristate is accomplished by combining Myristic Acid & Oxycodone Base.
It is the use of Oxycodone Base in Xtampza that causes it to come in the weird unrounded up dosage amounts that it does. This is due to Oxycodone Base being slightly more potent than when in its Hydrochloride Salt form. Which is the first of the couple other reasons that makes its use of Oxy Myristate different.
