I don't think 16mg is needed for. Someone who went on a bit of a run with Dihydrocodeine at doses of 300-500mg max

If I were you, I would skip the first day at the pharmacy. I understand, it's gonna suck to deal with withdrawal from the Codeine for an entire day. Be sure that precipitated withdrawal is going to be a much worse experience than waiting the 24 hours.
600mg Codeine is equivalent to 60mg Morphine in most Caucasian people. This isn't a crazy dosage or anything. It is significant though. If you make the wrong move and take the Buprenorphine too early, you're going to taste precipitated withdrawal followed by withdrawal for the rest of the day of a greater severity than had you just waited the 24 hour period.
The world of established medicine is still catching up regarding the proper application of Buprenorphine. I think it's great that they're doing it. From the perspective of a physician who doesn't understand dependence and withdrawal, I'm sure the idea of making an addict extremely sick during induction is not a big deal in the grand scheme of things. We all know that it would be significantly more difficult to apply Buprenorphine stepwise. I'm sure most folks would say "why not have one last solid reminder of how bad it can be" for good measure.
I really think this is your best course of action. You could even take a small amount of Codeine say, noon time if you'd theoretically be getting your medication in the AM the following day. You could take ~60mg Codeine just to help you with the worst of the rattles. I believe a small dose like this ~24 hours before induction would be fine.
I actually couldn't give a fuck now as I never rally went into detail about other drugs. The dihydrocodeine run was my snapping point coz that's only one that gave me euphoria AND the rattle, coz like you mentioned inwas hitting oramorph, Morphine pills, tramadol at doses of 2000mg on the trams easy.No your probably right.
