I personally wouldn't consider BDSM to be a 'fetish' for me.
You can couple it with sex or you can just do it on its own as an additive and that's how I see it. The arousal is different than being aroused for sex but it's fun getting your endorphins rushing for sex with BDSM.

Its extremely hot to know someones eyes are on you while you are playing with yourself especially if you dont allow them to touch you. But you could be talking about a third party watching. But I think this is more common then most would admit.

Imo the contributing factor that determines how good the sex is all depends on how well the anticipation is built. This begins long before you are even alone together.
