I have a doubt about mdma. Mdma is a basic drug, so in the stomach it will be highly ionized because of the hcl. Also a drug to reach central nervous system should be lipofilic and not ionized. How can be explained the strong effects on snc of a highly ionized drug? Thanks in advance.
 
Not everything has to make sense to be true nor scientificly proven to work. Just roll with it. Mdma is magic i dont care what science believes.

two words: chemical equilibrium.

in aqueous solution you'll always have an equilibrium between the salt and the free base with many molecules constantly changing from one form into the other (at equal rates when you've reached the equilibrium). in the stomach it's going to be pretty much on the side of the salt because of the low ph. in the blood it is shifted a bit towards the freebase in comparison (but still the salt should be something like 100x more common). because you still always have many molecules in freebase form that can diffuse through the blood brain barrier. these are removed from the equilibrium of the "body side of the barrier" which lead to the formation of more freebase. and once they've diffused through the hydrophobic membrane there's in aqueous solution again and you're more or less at the equilibrium for blood again.

of course is mdma would be more lipophilic or had a pka value closer to 7 it would pass the bbb even faster. all this is not specific to mdma, but is the same for most drugs that reach the brain. think of morphine, cocaine, lsd, atropine, fluoxetine, haloperidol, propanolol, metronidazole, ...
