Opioids Codeine combines with Tramadol?

Is it safe to take 120mg of codeine with 100mg of Tramadol every 4 hours? I heard they work differently and that the only risk is respiratory depression but there's not much information on it. Is there any risk of liver damage? thanks.

Safe? Not necessarily. In terms of respiratory depression you'll probably be fine if you're not very small or sensitive, as those aren't very large doses, but tramadol works for a little longer, and some people start feeling alarmingly spastic with even 300mg. So every 4 hours 3-4 times is probably still fine, but if you keep doing it every 4 hours after that, you may be asking for a seizure at some point. Lots of people do take higher doses but it is in no way recommended.
 
Taking tramadol every 4 hours is pointless. Tramadol itself is just a prodrug for desmethyltramadol(M1). It takes an average of 3 hours for M1 to reach Cmax, so it takes 3 hours to even get the full effect after oral tramadol.

Moreover, the half-life of tramadol itself is in the range of 5-7 hours, and M1 up to 10 hours(can't remember the exact figures).

Take them as you need then, but sticking to a regimen like that is unwise, and unnecessary.
