I feel for you. I was on methadone for pain and got to around 30mg/day. I heard horror stories about the WD so after about only 3 months I stopped. I was in a pain management clinic so they put me on MS-Contin (Morphine Sulfate) at what they figured was a similar dose of the methadone. Even with the morphine the methadone wd's hit around day 4.
I had some left over so I did a super fast taper. I went from 30 to 0 mg in about a week and a half. I had achy (like someone beat the shit out of them with a bat) legs for nearly 3 months and that is with another opiate on board. I can only imagine what it must be like for you by reading your posts.
You are right about the promethazine (phenergan). For some people it works but for me it was utterly useless. Zofran is probably the best anti-emetic I have used but it is damn expensive and not covered by my insurance.
You have probably already thought of it but dehydration can create its own set of nasty symptoms. You gotta keep the fluids going. If you can manage, whenever I have had a severe vomiting spree, I always visit the ER because they can do more for you than you can at home. Sounds like you've already gone but once you feel you can hold something down, I take a packet of top ramen mix (chicken), mix with enough water and nuke it.
You probably have all the help from the other posts you can get but I just feel for you and felt I should weigh in. Once off the stuff, never return. My docs always want to put me on it for pain because it works so well and is cheap but I will NEVER take it again.
I think someone mentioned something about Suboxone and that will certainly help just don't get hooked on that if can avoid it as I have heard the WD's are quite severe and protracted like methadone. I just re-read your OP so it looks like sub is out. If you can, continue tapering. I do not know how MMT works but if you want to avoid worse symptoms I would continue tapering until you are at a tiny dose.
With some opiates, and I have heard this about methadone, even the smallest amount will allow some relief. Jumping off, as you put it will no doubt be tough even at 4mg if you have been on it for an extended period of time. Oh yeah, watch out for addiction to benzos. That is something you definitely do not want to mess with dependency/addiction.
