Opioids Tapentadol, Hydrocodone, Tramadol, or Kratom (for pain)

If you had to choose between these four, which of these opioids would work best for analgesia without causing too many side effects?

I currently take 5-6g of Bali Kratom in the morning, combined with 5-10mg hydrocodone and 50mg tramadol, and by the time tramadol's O-Desmethyltramadol (basically Tapentadol) starts kicking in, the hydro and kratom begin to wear off, providing me with a consistent level of analgesia through the day with no peaks and valleys. I'll take 25-50mg of tapentadol or an additional 25-50mg tramadol for breakthrough pain on really bad pain days.

Low-dose buprenorphine has been suggested to me in another thread as an alternative to these, since I'm looking for an opioid with minimal euphoria (addictive potential) and decent analgesia, but I just don't see my doctor prescribing BuTrans or anything like that, just the ones I've listed (minus the Kratom, of course ;) ).

I took 200mg tapentadol for my back pain and experienced a very low control of pain, and a very messy feeling from a psychological point of view. You feel like to be on a opiate but all the euphoria is absent. I prefer the analgesic effect of dehydrocodeine 300mg in one oral shot, but it is mildly addictive so you end up with a need of having the 30mg pills always present. Not a good choice for daily use.

Hydrocodone. Easy answer. Try to get Norco 10/325 too.

I just wish hydro was a better analgesic for me, overactive 2D6 enzymes means it's elimated very quickly, it only gives me 2-2.5 hours tops of lackluster pain relief, and I've found hydro to give worse rebound pain (both physical and emotional) than synthetic opioids, like tapentadol do. It does mix very nicely with other opioids, though, gives a nice analgesic synergy when adding 5-10mg hydrocodone to kratom, tramadol, or tapentadol.

I agree about the messy feeling, which is actually a perk of tapentadol, imo. It makes it self limiting in terms of abuse and chasing the high, since any more than 150mg a day makes you feel like shit. Taking more than 75mg IR at once left me feeling like I had "medicine head" (bad), not euphoric (high) like hydro. I actually think it's a decently , more powerful than 10mg Norcos in terms of analgesia and opioid activity, and the lack of euphoria is welcomed in this case, since I'm not looking to get high here.

I'm just looking for sustained pain relief for as long as possible, and it seems like I'm on the best regiment available to me at the moment, unless I somehow get 0.2-0.4mg subutex tabs prescribed to me, or maybe BuTrans or low dose (5mg/day) methadone.

Right now, my pseudo-ER med is tramadol (O-Desmethyltramadol gives good analgesia for 10 hours with my 2D6 liver enzymes), and Bali Kratom lasts a solid 6-7 hours, much longer than hydro or tapentadol (my breakthrough meds) do.

Definately the Hydrocodone...beats the shit out of tramadol, kratom, and i can't say much for Tapentadol, but it also sounds weak.
Hydros are a little addictive, but its great for pain.

Go with a medium dose of your kratom with like 50mgs of tramadol, and use hydro for breakthrough. You might be overstimulation by tramadol but if you don't take too much kratom the synergy will be great.
