It's not really painful so long as you start carefully and build from there. You have to get used to it and learn to like it certainly, but that's not the same as painful. I think some of the critics are making a false assumption that straight men who do this are seeking a fake penis up the ass. Those people should google "prostate massage" and "prostate pleasure."
