Opioids Which Opiate Causes Least Histamine Release?

If you do have to switch to another opiate then, as others have indicated, it's kind of hit or miss. Each individual will react in their own way.

I would suggest that your first option be Hydrocodone. Since you did well on dihydrocodeine for so long. Hydrocodone would be the closest in strength and your MD would most likely be wiling to give it a try. If you go in asking for Dilautid I would think he may question your motives. Dilautid is notorious for drug seekers. Hydrocodone is a nice opiate. Also, Co-Tramadol is a great antihistamine and seems to work exceptionally well with Hydrocodone. You can get them together in a suspended release cough medication called Tussionex. It's very expensive though-if you don't have health insurance.

It would be excellent in your case though because it's similar to what you have been taking for pain-strength wise. It comes with a great antihistamine in it already AND it's suspended release (which doctors prefer to give for chronic pain vs. and immediate release)

I've had to take it several times for a cough and I've never had any itching from it-which I DO have if I take Hydrocodone alone. I have also had issues with edema myself-but never from Tussionex. OxyContin gave me severe generalized edema once. Scared me to death.

So, that's my .02. I would try asking for Tussionex and explain to your MD that it is extended release and has a antihistamine in it already. You may need a higher dose than a reg. pt uses for a cough, hopefully your MD can work that out with you. If you do need a unique dosage then you may have to get it from a compounding pharmacy. But, trust me, it's worth it.

Tussionex is my, and many others, favorite form of opiates. It's really like nothing else. The euphoria is out of this world, the mental and body effects are amazing. I would give anything for this to be the medication I used for my pain. I like it better than any other opiate I've ever taken. That's just me but, numerous other people seem to feel the same way.

I totally agree regarding tussionex. Causes excellent euphoria, I enjoy using it in conjunction with other opioids to keep from over-doing the chlorpheniramine maleate in it.

Its therapeutic index for controlling pain is probably somewhat narrow, but at lower dosages I think it might be a good option.
 
i had 100mcg IV fentynl earlier for a procedure in the hospital. 2hrs later i still have horrible itching. i suppose ill take some benadryl and see how that helps

i think that, the more you go away from the natural structura a.k.a. morphine/codeine to fully-synthetic opioids like fentanyl, piritramide, and so on, the more histamine effects decrease. oxycodone, oxymorphone, hydromorphone also have very little histamine effects. but they have them. you get the itches from time to time, but only very little ones, barely noticeable. fentanyl has no histamine reaction. methadone???

the more natural an opioid is, the higher the histamine reaction.

I have done a lot more research since posing the question and I agree with you totally.
 
A anti-histamine 45 minutes before any opiate ingestion will severely limit the amount of histamine release and provide some nice added sedation/noddiness
 
Tussionex sounds FANTASTIC - is it avaible in the UK does anyone know?

And also I totally take the posters' point about switching from one opiate to another of the same strength. My GP already thinks I am a total junkie from being on 300mg DHC (which from reading this forum I know is nothing). I went today and they have dished out two weeks' worth of oxycodone for me to try as it is a synthetic and less likely to cause allergies. I have been told to take 20mg a day in two 10mg doses.

I am wary of going up the scale of opiates. I love my DHC as it makes me feel mildly happy and gets rid of the pain. I have had to come off it in the past and my life was utter unredeeming hell (gave it up for 10 days and the withdrawal symptoms were as bad on Day 10 as Day 4).

Nope, unfortunately not. As far as I know, hydrocodone is not available at all in the UK (I never saw nor heard of it while living there.)

I may be wrong, but come to think of it I believe the US and Germany are virtually the only countries with hydrocodone...
 
^ Canada only has hydrocodone available in tablets/cough syrup to be prescribed as cough supressants, not for pain. I've always wondered why hydrocodone isn't more widely available, it's a very popular pain med in the US.

As for histamine release, morphine is the worst in my experience and the *codone family (oxycodone, hydrocodone) causes the least amount of histamine release, but this is based on solely my experience and no actual, factual data or clinical studies.
 
ill agree, now that ive thought about it for a couple of days....

hydrocodone makes me inch like a mother fucker
 
I never really had any itching problems with any opiates. i mean, sure I would itch sometimes after doing anice big IV shot of heroin, or oxy, or after doing a bunch of vicodin(etc..), but it never really bothered me whatsoever, in fact, I enjoyed it when it itched cuz it felt good knowing I was high enough on some opiates to get itchy. And then when I scratch it it feels even better, basically for the same reason I said above, but also because it just plain feels good to scratch an itch.
 
does anybody know if tramadol causes any histamine release? cause when i take 2 50mgs i get sooo itchy, not as bad as hydrocodone though
