Dosage MDMA, weight related for real yes or no?

Since I am sweeping Reddit subs frequently and dosage is one of the most frequently asked question:
Is weight really the or one of the most important factors for dosing MDMA?

I know that there is some dosage calculators and in almost every discussion someone throws in a key like X,XX mg x kg or lb. Now what is confusing me:
1. In any dosage recommendation on harm reduction sites there is a max dosage recommended. That doesn't align with an overall rule dosage by weight. Why should it be outruled just by crossing a fixed amount?
2. Dosage is about concentration in blood (and then ability to cross the blood-brain-barrier) as far as I know. Yes the amount of blood is aligned with weight, the more you weigh, the more blood. So a) are we talking in absolute % concentration of the substance and b) is the composition of the blood important meaning drinking a lot will spoil the rule?
3. I don't know any pharma medicine that is prescribed by kilo. Maybe the doctor will take your overall weight in account, but I have not seen it mentioned on any package leaflet in the pill box.

I would say that a extreme lean or heavy person should adjust the dosage but there is a broad span of body types which should hit the dosage. But this is pure feeling.

Side quest: A lot of 4-MMC users adapt the MDMA rule by weight. I personally do not see that. My wife is leaner than me and needs more of that substance. This is absolutely no data base of course.

Thoughts or facts? Rost me.
