Vivitrol and recent opioid use

I am new to this site and in DESPERATE Need of information on the Vivitrol shot(opiate blocker) for my 1st shot is scheduled for this Thursday (mid-day) and the last usage of H was tonight(Fri) and I plan on using Subs starting Tmr thru Sunday night. I have read Everywhere that u must be clean 7-10 days ahead of time before use of the Vivitrol shot. I have Also heard many rumors that if u are not clean for the recommended time, that u will go into severe remission of withdrawel..and if that's the case, Does anyone know how long the withdrawel will last for??? I am desperate for answers, and as I am sure you can see, also going into a state of panic. So, if any1 can HELP, Plz Do!!! :?

Regarding the vivitrol shot, since its an opioid antagonist (naltrexone), will rip any full agonist opioids off your receptors and send you into precipitated withdrawal which is just like opioid withdrawal but quicker hitting and more intense.

Buprenorphine however has a stronger affinity (as a mixed agonist-antagonist) so I don't know exactly how having bupe in your system when administering naltrexone will impact you. This is a little more advanced of a question so I'll send it over to the Other Drugs forum for you.

In the future you can find this post by looking for the thread titled 'Vivitrol and recent opioid use'. We encourage people to always search for answers so if in the future someone has the same question as you, they can find it easier with a more appropriately titled thread.

You posted in the "New Member Introductions" forum at first, and then theotherside26 (a moderator) moved your thread into "Basic Drug Discussion", which is a forum better suited for your question. Then, Cane2theLeft, another moderator, moved your thread to "Other Drugs", a place where your question will most likely be answered in quicker. In regards to your question, the Vivitrol shot is a shot of naltrexone, which is an opioid receptor antagonist much like naloxone (Narcan, the drug used to counter-act opioid overdoses in emergency situations).

Going from this information, I would say that you will have unpleasant withdrawal reactions if you are on opiates when the Vivitrol/naltrexone is administered.

However, because you were talking about Suboxone, which contains buprenorphine, I'm not entirely sure, because buprenorphine, unlike other opiates such as heroin, morphine, oxycodone, etc, is a competitive agonist, meaning it pushes other drugs out of the receptors and takes their place. I know that buprenorphine is powerful enough to even prevent Narcan/naloxone from entering the receptors.

I am not 100% sure if the same situation exists with Suboxone/buprenorphine and Vivitrol/naltrexone, as Vivitrol and Narcan are indeed different drugs with similar effects, those effects being blocking other opiates from reaching the opiate receptor.

I know this is a big long-winded post and everything, and probably doesn't cover everything, there are other people on this board who are more well-read (and well-experienced) with Suboxone and its interactions with other drugs, but I hope it gets you started in the right direction. :)

Hey OP,

If your last dose of opiates is heroin the Friday before the naltrexone you should likely not go into precipitated withdrawal as that is enough time for the heroin to dissipate. However regarding the buprenorphine as others have said naltrexone and bupe both have a strong affinity and I'm not sure what will happen.

My guess would be the naltrexone would not displace the bupe enough to cause any major precipitated withdrawal. This is only based on comparisons of naltrexone to naloxone and naloxone's weakened inability to reduce bupe induced respiratory distress, as well as people speaking about breaking through naltrexone blockades with bupe (though I may be misremembering the second one). Either way relying on that is not the safest course of action to take.

If you do end up taking bupe up to or anytime close to the injection please report back and let us know if you experience precipitated withdrawal symptoms though. It would be quite helpful information for future people in the same situation.

If your last dose of buprenorphine is Sunday night (tonight) and the naltrexone isn't until thursday, the buprenorphine shouldn't be a concern at all.

Not enough of it will still be bound and causing effects to make a significant difference, and as cane said above buprenorphine has a stronger binding affinity than naltrexone so it would only partially affect it its binding any how.
 
Hi, I really hope someone knows something; I have called every pharmacists, poison control, etc... and noone wants to answer my question and I have searched all over the web...

Anyways, my husband went into detox last week - he came out on Wednesday, last dose of suboxone .25 was Monday morning. His bed for rehab will not be ready until Monday (long story). On Wednesday he says he didn't relapse BUT some signs point to that he did; on Thursday morning 6am he took a 50mg naltroxine (he had tried to get clean on his own for the shot which is why he had the prescription in the house. Anyways, he went into rapid detox from either the rest of the suboxone or the herion he didn't use on Wednesday. He convulsed for about an hour then seemed like it was over, slept most of the day using other detox meds (trazadon, etc...) and woke up around 6pm puking, again seemed like everything was okay. He said it wasn't that bad. Then around 11:30pm he went to the bathroom and fainted and looked like he was ceasing for a minute (he says he didn't). He also said he passed out from low blood pressure form the sleep stuff that makes you dizzy.

Now it's the next day and I have no idea whether he should take another pill because the opiates are already retracted so the worst is over; or if he takes another pill the cycle will start all over.

Also I don't know whether the good out weighs the bad. He really wants to get sober, is going to the 10 ten "brain illness" rehabs in the country. I would prefer for him not to go through detox again, but if he doesn't take something he will use before Monday, I'm sure of it.

So question, should he take the 2nd pill? ANyone know what day 2 is like? or anyone know who, where or how i can find out. I obviously don't want him to do severe harm for not using for three days...

PLEASE HELP ME... I'm so concerned about my husband and I'm so supportive of his decision to get clean as he has decided this on his own. I can't believe the roadblocks we have faced, it makes me a non-user want to go get high, honestly.
