Tylenol Question

Hey all, I've been a regular Oxycontin user for almost 3 years now and just recently we hit a shortage in our area. Well, about a month ago I went to the dentist to have some work and was given Perc 10's for my pain, which I guess is a common thing dentists do.

Anyway, I didn't need them at the time and set them aside for a "rainy day", much like today and yesterday. For some reason I guess it slipped my mind for moment that Perc's contained APAP and now I'm worried I took too much. This is what I did:

I took 2 pills at around 6pm yesterday, then another 2 pills around midnight, and then I just took another 3 pills about 30 minutes ago. That's a total of 4,550mg of APAP within a 14-16 hour period.

I know the only thing I have going for me is the fact that I didn't take them all at once. Other things to note is that I do not drink, at all, and I do not take other acetaminophen containing products. In fact, when it comes to OTC pain relievers I prefer Advil to Tylenol.

So, with that said, should I be worried? I'm only 23 and would like to believe I have a fairly healthy liver. I've had workups done periodically from my doctors and no one has ever spotted a problem, but I don't know what kind of damage the Oxy has done to my live over the last 3 years.


Any quick advice is appreciated.

I know the recommended daily dose of Tylenol is no more than 4,000mg per DAY. They don't really specify an appropriate "hour range", they just say within a day. So I'd like to believe that (a) going over this by 500mg isn't a big deal and (b) as long I don't take anymore until, say, tomorrow then I should be fine.

The rub with acetaminophen is that everyone can be different enzymatically so that there have even been reports of damage at the 4000mg/24hr dosage. This had recently led an FDA panel to reccomend that the maximum dose level be dropped from 4000mg/24hr period.

At any rate, you're not going to drop dead from what you took. The worst that might likely happen is some liver damage, but it's honestly hard to know. If I were in your shoes, I'd take some SAMe (s-adenosyl-l-methionine) which is available from any pharmacy. It is a direct acetaminophen antidote. The hospital dosing regimen to treat a massive acetaminophen overdose is 2500mg every 4 hours for a total of 10000mg.

I take this stuff any time I consume acetaminophen. It couldn't hurt for you to try either. As long as you don't re-dose and don't drink alcohol anytime soon you should be fine.

I thought the antidote to acetaminophen poisoning was NAC?

NAC is an antidote, however it pretty must has to be given IV or in the inhaled Mucimyst form to be effective. Oral dosing regimens for NAC involve something like 40-50g orally which in and of itself causes nausea and vomiting, making the treatment useless.

SAMe is actually a bit more effective than NAC according to clinical trials, and is a better choice in many cases where oral dosing is needed. Plus, in those studies the effective doses were as I described above, 2500mg every 4 hours until 10000mg was reached.

I know in the UK they have a brand of acetaminophen called Paradote that includes methionine with the acetaminophen in case someone attempts an overdose.

I think you are probably alright at that dose since it was spaced at 14 to 15 hours. Also you do not indicate that this is a regular dosing regimen. I would think to just hold on until the 24 hour limit has passed before redosing, and since this is from a dental procedure you probably do not have a large quaninty of pills to begin with. Before I ever knew better, and should have researched it prior, I was taking 15 to 20 Norco a day for close to a year. That was giving me 4875 to 6500 mg's of APAP daily! Three subsequent liver function test's, over the past 5 years, though have found no problems, so I was lucky, VERY LUCKY!!!
