As per my thread earlier last week on switching from MS Contin to Tapentadol/Nucynta which has now been actioned by my doctor. I take 2x100mg XR tablets per day. The XR time release is 12 hours.
Conversion to mg for mg for opiate equipotency is hard, because Tapentadol relies way more upon its Norepinephrine Re-uptake Inhibitor properties than it's opioid affinity. Although it does have some level of opioid affinity, it is considered to be very week... Tapentadol is based on the active metabolite of Tramadol, O-desmethyltramadol, which used to get me very high, some people hate Tramadol but I used to love it. Massive rush at ~600mg with slightly dissociative properties at that does, and was the best high I'd ever achieved. I felt in utopia, a state of euphoria, nirvana... out of this fucking world.
A dose converter for pain relief (but not opioid converter, because Tapentadol has so little opiate action) says that 100mg of MS Contin or 40mg Oxycodone is equivalent to 200mg XR Tapentadol (which used to get me high if i'd stay off of opiates for a week or so). So Instead of spacing out the 2x100mg Tapentadol XR doses I took 200mg in one go expecting to be equally high as 40mg of Oxycodon or 100mg of MS Contin... but nothing. Absolutely nothing.
Admittedly, it's a very new drug and the information on it is sparse/unreliable. But I was just expecting a big daddy version of Tramadol, and was genuinely so excited to try it. As I've said, I took the equivalent of 100mg MS Contin or 40mg of Oxycodone which would have usually got me high after a week's break but I felt nothing, I wouldn't even know I'd have taken it.
Tapentadol costs a bomb so I'm being quite conservative with them, but tomorrow I'm going to try a single 400mg XR dose, so equivalent to 200mg of MS Contin or 80mg of Oxycodone IR and see what happens.
On a side note, it's been pretty shit with the pain to be honest, which is disappointing. So not only is it not helping with pain, but I can't bump the dose every now and then for a happy hour.
Any advice?
