Tagament And Opiods

I've read some posts on this board in regards to taking Tagament and opiods at the same time.
Why would you do that?
Does it make it lats longer?
Cut down on tolerance?
How much would you take (tagament) if you're used to taking about 3-4 Norco 10/325's a day?

Rumor has it that Ketamine, or even a 2-3x recommended dose of DXM before taking your opioids can decrease tolerance. The tagamet thing supposedly works because of cerain enzymes inhibited by the tagamet. Go to coldwaterworld.com for more info on this. However, I have read some things on this board that suggest that tagamet actually reduces the effictiveness of your pills. I know that this is definitly the case with codiene, but don't know about hydrocodone.

Ok so tagament=cimetidine apparently inhibits the CYP enzymes specifically CYP4502D6. Unfortunately this is the enzyme primarily responsible for the conversion of codeine to morphine (also for hyrocodine to hyromorphone and oxycodone to oxymorphone but in this case it may be the parent compound with the most activity). Thus it is definatly bad with codeine, or questionable utility with say oxycontin (it might be usefull might not..but probably not that significant...in fact the following study found no differnce for CYP2D6 metabolism of oxycodone. oxycodone. ). So at best it is a waste of time and at worst a high killer for these opiates.
Now for other opiates it is less clear. If morphine/fentanyl/heroin is metabolized by a CYPP enzyme it might be usefull otherwise it is probably irrelevant. Anyway cimetidine isn't a very powerfull inhibitor (could be usefull for shot acting benzos but that is about it...mostly because nothing else common inhibits CYP3A4) so you probably aren't going to benifit much.
