Sending stuff to pilltest.com

Here's my prob, i'm considering sending a very weird tab off to pilltest in Amsterdam (using a courier company), but I'm uncertain what to put on the forms for customs? Hmmm...
 
For those that don't know. They will not accept anything from out of the country. It is illegal by Dutch law.
